HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The incumbent in this position functions as the Case Manager for Salem VAMC's Housing and Urban Development/ VA Supportive Housing (HUD-VASH). The HUD-VASH Program falls within the Healthcare for Homeless Veterans (HCHV) Program of the Social Work Service

Duties

DUTIES: GS9: GS-9 is the entry level grade for the GS-0185 Social Work series and is used for Social Workers with less than one year of post-MSW experience and who are not yet licensed or certified at the independent practice level. The incumbent is responsible to arrange ongoing supervision for licensure qualification and must pass the licensure exam within the specified time frame to maintain his/her position with the Salem VAMC. The incumbent is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. The incumbent performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. The incumbent coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Incumbent is responsible for transporting Veterans when said transportation is consistent with the Veteran's recovery plan (i.e., to address medical appointment, food insecurity, exploration of housing options, etc.). In order to perform this function, incumbent is required to maintain a valid driver's license, routinely pass a driver's physical through Employee Occupational Health (EOH) at intervals determined by EOH and complete all required VA trainings related to driving a government vehicle. The incumbent will provide a full range of social work services within commonly accepted standards of social work practice, which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. The incumbent actively participates in the HUD-VASH Program conference calls and attends trainings offered by the HUD-VASH program. The incumbent is responsible for following local policy and SOPs for the HUD-VASH Program, Social Work Service and the VA Medical Center. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation. Attend all appropriate staff meetings (i.e., interdisciplinary treatment team meetings, Social Work staff meetings, Social Work supervision meetings, and other committees and workgroups, as appropriate) and perform other duties as required. GS11: Social Worker has a master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). The GS-11 is the full performance level for Social Work and licensure, or certification is required in a state at the independent practice level. The incumbent is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. The incumbent performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes The incumbent verifies the Veteran's eligibility status for VA Medical Care and program participation. The incumbent is responsible for ensuring that homeless Veterans are considered primary participants for program acceptance. The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by the HOMES evaluation procedures. The incumbent is also responsible for providing HUDVASH case management Incumbent is responsible for transporting Veterans when said transportation is consistent with the Veteran's recovery plan (i.e., to address medical appointment, food insecurity, exploration of housing options, etc.). Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. [Entry Level] Assignments. Individuals assigned as GS-9 grade level social workers are considered to be at the entry level and are closely supervised, as they are not yet functioning at the independent practice level conferred by independent licensure or certification. Social workers at the GS-9 entry level are typically assigned to program areas that do not require specialized knowledge or experience. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Duties include but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills and knowledge to maintain Veteran privacy and confidentiality per policies, handbooks or directives; and acts as an advocate with appropriate VA and community service providers and agencies when it serves the best interest of the Veteran and family members/caregiver. Incumbent independently assesses the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity. In collaboration with the Veteran, family/caregiver, and interdisciplinary treatment team, the social worker facilitates the delivery of health care services. The social worker identifies family/caregiver stressors, conducts assessment and provides specific interventions. The incumbent provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice.
