This position is located within the Diagnostic Imaging Service, Ultrasound Section, at the Loma Linda VAMC. Medical Instrument Technicians (MIT) at this grade level serve as staff MIT's and are to be qualified to provide services in specialized areas of sonography. The MIT operates independently and without direct supervision as the technologist is responsible for the performance, accuracy, and coordination of all ultrasound examinations, both general and vascular.

The sonographer must have a full knowledge of normal and abnormal anatomy, and must be competent in the production, use, recognition, and analysis of anatomical structures of the bodies' organ systems. The sonographer is regarded as the expert source of all ultrasound imaging, and the interpreting Radiologist relies heavily on the skills, knowledge, and abilities of the sonographers when they provide a final interpretation. Duties include, but are not limited to: Sets up and adjusts all equipment and manually maneuvers the transducer to best demonstrate normal and abnormal anatomy. Performs a wide range of Diagnostic Abdominal Ultrasounds examinations, ultrasound examinations of the female pelvis, and non-invasive Vascular Ultrasounds. Understands the principles of ultrasound and the highly specialized equipment to be able to utilize the equipment to its highest potential. Controls technical factors on ultrasound unit, which will affect time, gain, amplitude, velocity, frequency, attenuation, absorption, reflection, scattering, refraction and diffraction. Advises Radiologist or referring physician of results of exams through verbal communication and Technologist notes. Assists in the overall workflow of the department by coordinating and organizing the daily patient schedule to expedite workflow within the department. Receives and identifies patients, explains methods of procedures, assists movement of patients to and from wheelchairs, carts and tables, positions the patient and performs the examinations requested. Properly identifies patient images and exams per JCAHO and VHA requirements. Assists Radiologist and other Physicians during ultrasound guided biopsies, drainages, and aspirations. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. Licensure or Certification: Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under "Titles and Certification/Registrations". For all specialties identified in this standard, Basic and Advanced Cardiac Life Support certifications are desirable. Training which does not result in official certification does not meet this definition. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-9 Experience. At least 1 year of experience comparable to the next lower grade level (GS-08), which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be specialized experience which demonstrates possession of the knowledge required in order to provide all aspects of diagnostic ultrasonography services. Examples of experience equivalent to the GS-08 grade level include but are not limited to: perform diagnostic ultrasound examination on multiple areas of the body, including, but not limited to, the abdomen, pelvis, transvaginal, chest, small parts, thyroid, breast, and scrotum, perform vascular studies such as vertebral, renal artery, aortic aneurysm, portal- systemic shunts, etc., assist radiologists with biopsy procedures determining the location, depth, and required needle angle for the lesion to be sampled, and assist in surgical procedures and fluid aspirations. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs: Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, and 3 are appropriate for all positions at this level. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols that pertain to diagnostic ultrasonography. Knowledge of JCAHO and other regulatory requirements. Ability to communicate effectively and professionally with employees at varying grade levels. Certification. Registration by Cardiovascular Credentialing International (CCI) or by the American Registry of Diagnostic Medical Sonographers (ARDMS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. Special Assignments. Non-supervisory diagnostic ultrasound technicians may have assignments that because of their nature substantially exceed the full performance level. The Professional Standards Board will review proposed non-supervisory special assignments for the GS-9 level and make a determination regarding their appropriateness. These assignments are expected to be relatively few in number and must represent substantial additional responsibility over the full performance level. Physical Requirements: The physical demands of the work require extended periods of sitting, moderate walking, and standing. Work involves constant bending, lifting, twisting, and carrying moderately heavy items, or moving equipment around, such as the ultrasound machine. The work requires moderate lifting, usually less than 50 lbs. The work requires pushing wheelchair and stretcher patients. Positioning patients who are disabled on the stretcher requires considerable effort and working in awkward positions. Lifting of totally incapacitated patients is done only with the help of lifts and/or other employees.
