The Social Worker in this position is the Coordinator for the Contract Nursing Home Program and Adult Day Health Care program, managing administrative and clinical aspects of the programs across two major campuses and multiple CBOCs, as well as, managing the daily operations of the programs, developing policies and procedures for the programs operations and preparing reports and statistics for service line, facility, VISN, and national use.

Duties included, but are not limited too:
Utilizing funding data related to the program regarding day to day functioning and program evaluation.
Responsible for coordinating interdisciplinary staffing, work assignments and admission criteria of the program.
Providing direct clinical services through visitation to contract nursing home residents and conferences with facility staff to address and resolve patient concerns as they develop and ensure services are provided with attention to dignity of resident and age-related capacity to understand treatments.
Serving as the Coordinator for the Adult Day Health Care Program.
Collaborating with the Director, Care Management Service Line in policy development and with Patient Services regarding funding expenditures.
Tracking resource utilization and resource allocation decisions in participation with the ADHC Committee.
Hold an active, current license at the advanced practice level.
Responsible for assuring that monthly follow-up to Veterans in the CNHs are completed as required by the program.
Responsible for assuring, in concert with GEC that the placement of veterans into the CNH program meets VA eligibility requirements.
Assuring the annual VA CNH and program reviews are completed as set forth in VA policy and must have a good knowledge of Centers for Medicare/Medicaid standards concerning CNH programs.
Serving as the Contracting Officer Representative for the CNH program.
Responsible for assuring that quality assurance related to these programs are within program parameters.
Responsible for assuring the reporting and investigating of sentinel/adverse events by program staff as necessary, and for chairing the CNH Oversight Committee.
Maintaining a current list of active participants in the program to enable tracking of service activities, utilization of services and other functions.
Providing administrative, technical, and educational consultation necessary for accomplishing the work of the program, including education to health care system staff across the health care system and outreach efforts to community programs such as the state ombudsman program.
Resolving informal complaints and grievances.
In the capacity as Coordinator for the Adult Day Health Care Program, the incumbent reviews requests for authorization of veterans to determine whether they meet eligibility criteria for inclusion in the ADHC program and is responsible for the development, evaluation, and communication of standards, policies and procedures to health care system social workers who serve as primary referral sources for the program.
Providing liaison with the participating ADHC providers and providing program oversight to ensure quality care is provided by contracted agencies.
Performing a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, using sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements.
Performing program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.).
Performing work related to the review, monitoring, analysis, planning and assessment of various programs.
Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer.
Performing other related duties as assigned.
Responsible to the Assigned Supervisor but practices independently, exercising initiative and independent judgment in day-to-day activities.
Delegated the responsibility and authority for the planning, direct staff and coordination of all elements which constitute the CNH program and the ADHC Program. This requires a high and varied level of professional expertise and collaboration to develop, revise and/or apply policies and standards which determine the scope of these programs.

Qualifications
Basic Requirements:
Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the 3 accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.

Grade Determination - Social Worker (Program Coordinator), GS-12
Experience & Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, & Abilities (KSAs). In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.
Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.]
