The Battle Creek VA Medical Center is hiring an advanced practice social worker to serve as a Suicide Prevention Coordinator for the Suicide Prevention Program. This position is aligned in the ACOS of Mental Health's service line and provides program responsibilities to all locations within the Battle Creek VA network. This position may be filled at either Muskegon or Wyoming.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Major duties of this position include but are not limited to: Functioning as the coordinator of care for Veterans at suicide risk receiving care through the Battle Creek VAMC healthcare delivery system including its Outpatient Clinics. The SPC also serves as an advisor to the Associate Chief of Staff for Mental Health, Mental Health Service Chiefs, facility leadership, program managers and other medical center staff concerning suicide prevention strategies for individual patients, for the facility and for the Veteran community as a whole in conjunction with the SPCs at the Wyoming Health Care Center and Battle Creek VAMC, and other members of the Suicide Prevention Team. Specific functions of the SPC include, among others, identifying Veterans at risk for suicide, tracking appointments and coordinating enhanced care as needed, educating providers, Veterans, families, and members of the community about risk factors and warning signs for suicide, and identifying treatment options. This is accomplished through: Tracking all suicide attempts and patients with serious suicide ideation at each facility through the use of the "Suicide Behavior Report" progress note. The coordinator will implement the use of this template at each facility and be placed as a co-signer or additional signer on the template in order to be made aware of all attempts and patients with serious ideation. Ensuring that Suicide Behavior Flags are posted on the charts of relevant Veterans and monitoring the need for continued flagging. Assuring that all Veterans on the facility tracking list are being followed closely by the appropriate provider on a regular basis as determined by the coordinator and the provider. Tracking any no-show appointments by these patients and following up with the patient as soon as possible to reschedule the appointment. Following up with Veterans who have contacted the Veterans Crisis Line. Maintaining required databases and reports per the National Suicide Prevention Program. Preparing Heads Ups and Issue Briefs as needed. Monitoring and maintaining the REACH Vet database and the clinical care required. Monthly contact with Veterans deemed at high risk for suicide as defined by national guidelines and with those Veterans who remain actively on the high-risk list. Monitoring the completion of the treatment plan, safety plan, and means reduction for those patients who have been identified to potentially pose an increased risk for future suicidal behavior. Disseminating, organizing, and, where possible, conducting evidence-based and promising, emerging interventions. Maintaining awareness of suicide treatment options and making recommendations to the patient's provider when new options become available. Obtaining any relevant HIPAA clearances to allow transmittal of facility data on suicide attempts and individuals at risk to the Suicide Prevention Center of Excellence for trending and quality assurance purposes; and transmitting the data following cyber security requirements as applicable for this sensitive data. Assuring that all of these patients have a 24-hour resource number including the VCL to call and ways to get assistance if an immediate crisis should occur. Establishing an on-going Gatekeeper Training program with non-clinical frontline staff such as clerks and telephone operators to assure that they are aware of options for patients with whom they come in contact during crisis situations. Working with community resources such as VSO's and other community groups to assure that they are aware of how to recognize Veterans at risk for suicide and their options for getting these Veterans assistance. Developing and implementing an ongoing education plan for providers, Veterans, families, and members of the community on risk factors and warning signs for suicide, and treatment options. SPCs will participate in training activities and scheduled conference calls as required by the Office of Mental Health and Suicide Prevention (OMHSP) Performs other related duties as assigned Due to the nature of this position, regular travel to CBOC and other community locations will be required. This travel will generally originate from the home station during normal business hours. Qualifications

BASIC REQUIREMENTS. Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Physical Requirements. Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. The Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the Social Worker or others. Position requires driving to sites of care, standing for extended periods of time, and involves sitting at a computer, in a car, and in counsel and consultation with Veterans and interdisciplinary teams. This position requires close visual acuity to perform an activity such as: viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers. English Language Proficiency. Candidates must be proficient in spoken and written English Grade Determination: Social Worker (Program Coordinator), GS-12(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(e) Ability to provide training, orientation, and guidance within clinical practice.
