This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Social Work Program Coordinator (MH RRTP Admissions Coordinator/Compact Act Coordinator) serves in two distinct roles, the duties include but are not limited to the following: Working with members of a multi-interdisciplinary MH RRTP team, responds and completes consults and other tasks related to MH RRTP prescreening/screening/admissions/care coordination. This requires familiarity with and adherence to complex and sometimes competing locally and nationally defined eligibility requirements and admissions processes. Manages MH RRTP admission applications and interviews, ensuring they are reviewed timely by a designated screening and admissions team, of which incumbent is a central part. Veterans must be admitted according to the Veteran's objectively known needs for services and the MH RRTP's ability to provide those services. MH RRTP AC/CAC Social Worker maintains clinical contact will all Veterans while awaiting admission to the MH RRTP. The MH RRTP AC/CAC Social Worker assists participants, families, and clinical team members in coordinating care in the community and resources when clinically indicated and works in unison with multi-disciplinary team in promoting a Veteran-centered admissions and discharge culture that assures quality health care services are provided to the Veterans promptly, safely, courteously, and compassionately. The MH RRTP AC/CAC communicates effectively both orally and in writing with professional, administrative, and technical staff at all levels. Functions as liaison between MH RRTP's, hospital staff, emergency departments, internal and external resources including community resources/courts/probation and parole officers, guardians, conservators, fiduciary and other providers concerning appropriate notifications. The incumbent performs a wide range of services and provides technical assistance to state, local, and private agencies, and universities. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program and admission process enhancements. In the COMPACT Act Coordination role, the incumbent will engage/serve through: Utilize program coordination skills to develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services Oversees daily operations of the program and provides reports and metrics to facility, VISN and VACO as requested Navigation between varying levels of care Coordination between health care providers and care managers in direct care and community care Comprehensive transition planning Outreach to community providers and healthcare systems providing COMPACT care Advocacy for Veterans, ensuring they do not receive bills for COMPACT related care A singular point of contact for Veterans, families and caregivers during COMPACT episodes of care Facility subject matter expert for COMPACT Act Utilizes designated COMPACT Act templates and coding requirements when documenting in the medical record Coordinate with local Patient Advocate Tracking System - Replacement (PATS-R) COMPACT clinical review teams Collaborate with their local facility revenue and finance departments Coordinate with their local facility Office of Integrated Veteran Care (IVC) Coordinators Work Schedule: Monday to Friday | 8:30am to 4:30pm EDRP Authorized: Former EDRP participants ineligible to apply for incentive. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. BASIC REQUIREMENTS 1- United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. 2 - Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to https://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. 3 - Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to https://dvagov.sharepoint.com/sites/social-work-matters/SitePages/Social-Work-Licensure-by-State.aspx May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GRADE DETERMINATION - GS-12 1- Experience/Education. One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. 2- Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure 3- In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. The actual grade at which an applicant may be selected for this vacancy is GS-12. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospital, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. The incumbent may carry and lift light items weighing less than 15 pounds. The incumbent must possess a current Driver's License and drive a government vehicle in carrying out professional duties when deemed necessary.
