The CRH Nurse Manager, Primary Care & Population Health (CRH NM, PC-PH) serves as a first level supervisor who functions under the direction and guidance of the Deputy Chief Nurse, VISN 21 CRH. S/he primary responsibility is to lead the delivery of the PACT models of care being provided by staff under his/her supervision by reviewing, assessing, and measuring outcomes related to the processes involved in preventive & disease management that is patient driven and utilizes a team-based approach.
Duties
General Description of Assigned Duties:
Nurse 1, Level 1: Demonstrates the fundamental knowledge of the Clinical Resource Hub (CRH) and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
Nurse 1, Level 2: Demonstrates integration of biopsychosocial concepts, cognitive skills, and competent practice and knowledge of the Clinical Resource Hub (CRH) and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
Nurse 1, Level 3: Demonstrates specialized knowledge of the Clinical Resource Hub (CRH) and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
Nurse 2: Functions as a clinical expert within the Clinical Resource Hub (CRH) and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
Nurse 3: Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. They demonstrate sound decision-making and judgment and possess excellent interpersonal skills. Functions as a clinical expert within the Clinical Resource Hub (CRH) and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care.
Assumes responsibility and supports the Veterans Integrated Systems Network (VISN) CRH under the guidance of the Veterans Health Administration (VHA) to provide health care services to Veteran patient across the continuum of care in both institutional and non-institutional settings. Provides critical medical, technical, and program support necessary to sustain efficient telehealth operations for the VISN CRH. Exercises leadership and organizational skills, independent judgement, problem-solving ability, and expertise in program implementation and evaluation. Influences care outcomes by collaborating with members of the interdisciplinary team and participating in process improvement initiatives.
Serves as a first level supervisor who functions under the direction and guidance of the Deputy Chief Nurse, VISN 21 CRH. S/he enhances the development and implementation of the Primary Care and Population Health Programs by working in partnership with their CRH leadership as well as with Primary Care and Population Health Leadership at the spoke sites. The CRH NM, PC-PH demonstrate leadership, experience and creative approaches to work independently optimizing patient care, patient safety and operational efficiency for patients receiving care within the Primary Care and Population Health Programs. The NM will demonstrate expertise in leading and organizing care delivery, staff accountability for practice, promotes staff engagement in decision making and guiding and supporting staff growth and development. S/he utilizes evidence-based practice to develop and evaluate program and service activities that impacts positive patient outcomes within CRH Nursing Service Primary Care and Population Health. The CRH NM, PC-PH primary responsibility is to lead the delivery of the PACT models of care being provided by staff under his/her supervision by reviewing, assessing, and measuring outcomes related to the processes involved in preventive and disease management that is patient driven and utilizes a team-based approach. This position is aligned under CRH Nursing Services, but work collaborative with all CRH Primary Care and Population Health Leadership to meet the mission and vision of VISN 21 CRH. This position requires cross-coverage in the absence of another Nurse Manager in CRH.

Qualifications
Basic Requirements:
English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.

Nurse II

MSN and 2 years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Master's degree in *related field with BSN and two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in Nursing with and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.

Salary for this position ranges dependent upon applicant qualifications
Grade 1: $149,816-$199,250 annually
Grade 2: $172,288-$221,900 annually
Grade 3: $189,516-$221,900 annually
Preferred Experience:
3 to 5 years of Nurse Manager Experience
2 to 3 years of Primary Care and/or Population Health RN Experience

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The position requires potentially long periods of combined walking, standing, stooping, sitting, bending, pulling, and pushing.
Other Significant Information: The incumbent must be a mature, flexible, sensible individual capable or working effectively in stressful situation, able to shift priorities based on needs of the organization. The incumbent who provides off-site training may be required to travel occasionally. This position potentially requires flexibility in schedule and assignments.
