The incumbent functions as a mid-level practitioner and works with assigned providers and health care team(s) in designing, implementing, monitoring, evaluating, and modifying patient pharmacotherapy. The Clinical Pharmacist Practitioner is responsible for developing, organizing, & managing multiple complex programs.

1. General Description of Assigned Duties.
a. The Clinical Pharmacist Practitioner (CPP) practices in a major clinical area (primary care, psychiatry, medicine, geriatrics, etc.) in the health care organization. The incumbent functions as a mid-level practitioner and works with assigned providers and health care team(s) in designing, implementing, monitoring, evaluating, and modifying patient pharmacotherapy.
b. The Clinical Pharmacist Practitioner is responsible for developing, organizing, and managing multiple complex programs (i.e. consultative, therapeutic drug monitoring, collaborative drug therapy management, quality assurance/utilization review, formulary management, drug information, etc.) for their assigned health care team(s) and Pharmacy Service.
2. Functions or Scopes of Practice.
a. Clinical
1. Evaluates and monitors medications for appropriateness of therapy and patient's response to therapy based on relevant therapeutic endpoints by means of patient interviews, physical and functional assessment, and diagnostic tests. Requests consultations or referrals to other health care professionals to render appropriate comprehensive care to the patient. Prescribes and modifies medication regimens in a safe, effective, and economical manner. Provides these collaborative drug therapy management services in accordance with the individual's approved scope of practice and prescriptive authority.
2. Evaluates drug therapy through direct patient involvement, with clinical assessment and objective findings relating to patient's responses to drug therapy and communicating and documenting those findings, interventions, and recommendations to appropriate providers and in appropriate records (e.g. patient's medical record). Documentation is done in a timely manner and consistent with the department's established standards of practice.
3. Prescribes, continues, modifies, or discontinues drug therapy for both inpatients and outpatients in accordance with the individual's approved scope of practice and prescriptive authority.
b. Education
1. Maintains professional competency on drug therapies and innovations in pharmacy practice pertinent to the use of anticoagulants.
2. Provides medication counseling to patients and/or caregivers. Evaluates the abilities of adult and geriatric patients to understand medication instructions (i.e. ability to learn, sensory limitations, past knowledge of condition, and obstacles to following treatment regimen) and provides oral and written counseling on their drug therapy.
3. Assists in the development, coordination and provision of drug therapy in service programs, staff development programs, patient education programs, and other system- wide drug information programs.
c. Anticoagulation Program Management
1. Serves as a leader and subject matter expert in the oversight, design, implementation, and function of the anticoagulation management program.
2. Participates in quality assurance and medication use evaluation programs particularly as they pertain to anticoagulation therapy management and reports to relevant Health- System committees (e.g. Pharmacy and Therapeutics committee).
3. Manages the centralized Anticoagulation Clinic at VA Eastern Kansas Healthcare System (Leavenworth and Topeka divisions).
d. Administrative
1. Performs proper recordkeeping and maintains proper documentation when required or appropriate for all clinical activity and patient interventions.
2. Ensures sensitive data for patient files, both printed and electronic, remain confidential. Ensures ADP systems for pharmacy practice and patient care are utilized for official duties and remain secure.
3. Provides professional guidance and leadership to their assigned work team(s) to deliver patient care services in accordance with established standards of practice (i.e. DEA, VHA, JCAHO, health care system, departmental policy & procedures, applicable state laws, etc.) and in support of established VHA and VISN 15 performance measures.
e. Research
1. May sign and/or participate in research studies under FDA guidelines that have undergone review and approval by the Research & Development Committee and the Investigational Review Board.

Qualifications

Basic Requirements:
Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the 1 graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Preferred Experience: Completion of PGY1 Residency program; Ambulatory Care experience; Anticoagulation Medication management experience.
Assignment. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages 5 single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc.; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacist assigned to this position must demonstrate the following Knowledge, Skills, and Abilities (KSAs):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
Expert understanding of regulatory and quality standards for their program area.
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-13.
