Robley Rex VA Healthcare System, Louisville, Kentucky is currently recruiting for a Licensed Practical Nurse Scheduler in the Operating Room Clinic.
Duties
The OR Scheduling Licensed Vocational Nurse provides nursing care which requires the knowledge and skills of an individual licensed as an LPN/LVN. The OR Scheduling Licensed Vocational Nurse maintains a high level of practice under the supervision of a Registered Nurse. The LVN is responsible for the quality of care delivered and promoting personal and professional behaviors.
Individualized Patient Care- Manages patient care problems and deviations using self-direction in accordance with VA Nursing Practice and LVN licensure standards. Maintains knowledge and skill to care for a wide range of patients, including those with chronic and acute illness. Demonstrates comprehensive understanding of disease conditions, nursing principles, and standards/practices to coordinate nursing care required in the clinic setting and alter the sequence and/or care delivered to meet individual patient's needs.- Observes, records, and reports vital signs, orientation x3, weight, and blood glucose levels. Appropriately triages and prioritizes patient needs as routine, urgent, or emergent in the clinic setting and modifies patient care/treatment as needed.
Administrative - Completes all required documentation in a timely manner and in accordance with VA policy and TJC standards of care. Demonstrates excellent written and oral communication skills.- Remain current on all changes, improvements, and software patches related to VISTA and CPRS systems. Demonstrates daily/weekly use of VA resources, such as VSSC and Data Warehouse to assist team with panel management, which includes identifying high-risk patients, ensuring 2-day post hospital discharge contacts are made, determining patients in need of annual vaccinations or lab tests, etc. Serves as a resource for coworkers regarding the use of these systems.
Medication Administration- Prepares, administers and documents medications and immunizations as directed without error that may cause harm to patients. Demonstrates practical and theoretical knowledge about medication side effects, purposes, therapeutic dosage range, and toxic effects with skill sufficient to administer a wide range of medications.- Demonstrates advanced knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications permitted by facility policies and procedures and within the LVN Expanded Scope of Practice. Examples include administering immunizations, influenza vaccinations, Zoladex, and Eligard injections. Performs a learning needs assessment and provides individual teaching for self-administration of specific medications. Recognizes subtle signs and symptoms of drug interactions and promptly notifies provider.
Customer Service- Demonstrates excellent internal and external customer service skills, including face-to-face interactions, telephone communication, secure messaging, and instant messaging. Possesses patient advocacy skills to resolve any potential issues as they occur during clinic visits.- Maintain courtesy and professionalism with patients and family members under stressful situations which requires explanation, education, and instruction of procedures, treatments, disease processes, etc.… Exercises skill in communicating with patients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions. This includes the ability to effectively deal with the above types of patients under very general supervision.
Population Specific Skills- Obtains and documents cognitive, physical, emotional, and chronological patient attributes.- Adheres to common practices of age specific needs according to age related factors based on the mental and physical conditions of patients.
Training and Education- Identifies patients' learning needs and readiness for learning. Plans and implements appropriate teaching strategies based on assessment. When needed, initiates consult to specific educators.- Completes mandatory Medical Center and professional training requirements in a timely manner. Properly documents all educational activities. Collaborates with Talent Management System (TMS) Coordinator when needed to address educational requirements.
Safety and Security- Adheres to all safety procedures to avoid preventable injury or harm to patients, family members, visitors, and/or government property.- Independently identifies and reports infection control issues and environmental hazards in a timely manner. Intervenes appropriately to ensure safety.

Qualifications
GS-7 LPN: This is above full performance level for this occupation

Basic Requirements:
Citizenship: Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the National League for Nursing Accrediting Commission, 61 Broadway, 33rd Floor, New York, NY 10006. License: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: The GS-7 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment.
(a) Completion of at least one (1) year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all of the performance requirements for the GS-6 LPN/LVN.
(b) Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Knowledge base promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Positions at this grade level require broader knowledge and skills because supervision is more general and employees are expected to be involved to a greater degree in the development and modification of treatment plans.
(c) Ability to assist RN and/or MD/DO to appropriately prioritize overall patient care needs and adjust plans for care delivery. Makes astute observations of subtle, less obvious changes, both physical and emotional, in patient's condition, and accurately and thoroughly documents care delivered and patient's response to care that could require immediate modification of the patient's care plan. Appropriately seeks assistance and guidance by informing RN or MD/DO of changes in patient's condition requiring higher-level intervention. At this grade level, the LPN/LVN is expected to participate to a greater degree in prioritizing, and, as appropriate, modifying patient care treatment. They must also possess the ability to observe more subtle physical and emotional changes in patients and demonstrate taking appropriate action based on those observations.
(d) Ability to proactively assist the RN or MD/DO in addressing patient's needs for medication/treatments, accurately administering prescribed medications/treatments according to established policies/procedures and appropriately recording administration and patient's response to medications/treatments received. (This may include restricted medication locally authorized for administration by employees at this grade level.) Demonstrates the knowledge and ability to readily recognize the more subtle signs and symptoms of potential or actual drug reactions, promptly inform the RN or MD/DO of observed changes in patient's condition and initiate appropriate, prescribed interventions. Employees at this grade level demonstrate broader knowledge and skill with respect to these responsibilities than employees at lower grade levels.
(e) Knowledge and skill to promptly recognize potential urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO for appropriate team intervention and effectively intervene, as directed, to assure the continuation of optimal, safe and therapeutic patient care delivery. At this grade level, the LPN/LVN must have a broader knowledge of the patient treatment process and exercise judgment that demonstrates the ability to effectively intervene when appropriate.
(f) Assists individuals or groups of patients and their families to take an active role in promoting healthy lifestyles for more positive patient outcomes. Exercises skill in influencing and communicating with unusually difficult to care for or communicate with patients and/or family members who may exhibit problems with lack of self-control, resistant or abusive behaviors, or impediments in their ability to understand or follow instructions. This includes the ability to effectively deal with the above types of patients under very general supervision.
Preferred Experience: Previous experience in a surgical setting preferred. Experience in outpatient procedure areas preferred.

Physical Requirements: It is VA policy to ensure that all selected applicants and employed personnel are physically, mentally and emotionally fit to perform the duties of the position to which assigned. A pre-placement physical examination is required for all fulltime, part-time and intermittent employees identified by VA Directive and Handbook 5019. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on organizational needs. Must complete annual Employee Health requirements, such as annual Influenza vaccination, TB screening or testing, as a condition of employment.
