This position is organizationally aligned under the Geriatrics and Extended Care Service Line (Home & Community Based Care Programs) at the Jesse Brown VA Medical Center in Chicago, IL. The purpose of this position is to provide administrative support to the manager and staff.

Duties

Major Duties include: Request medical documentation for all five community health services (over 4,000 document requests each year) verbally and in writing; and performs follow up calls when documents are not forthcoming. Documents agency contacts (three times minimum) in CPRS Consult Toolbox. Receives all documentation (Home Health Certification and Plan of Care -- CMS 485), verbal order confirmations, change orders, etc.) from home care agencies in the community requiring physician signatures. Sorts and distributes documents to physicians across the medical center and community-based outpatient clinics (CBOCS). Maintain tracking sheets to document receipt, distribution chain, and return of documents from agencies and physicians. Completes GEC consults by completing the appropriate note in CPRS consult toolbox and scanning all Home Health Certification and Plan of Care - CMS 485 documents that include physicians' signature into the Vista Imaging program assuring the reports are linked to the appropriate consult, progress note title, and patient. Ensure documents are returned to the agencies as appropriate. Creates and maintains budget spreadsheet for tracking Veterans Directed Care spending plan monthly. Maintain all documents generated by the third-party administrator and scans reports into the patient chart when the patient passes. Facilitates weekly interdisciplinary staff meetings by designating patients to be reviewed, ensuring clinical staff remains on task, and taking minutes. Transmits encounter data for HBPC monthly to ensure workload is tracked and reported for average daily census. The incumbent provides the Home Care Manager with weekly statistical data on the types of visits, number of veterans seen, admissions, and discharges. Ensures office supplies, equipment, and furnishings authorized are ordered and received. Employee must acquire knowledge of internal organizational procedures, knowledge of various facility services, knowledge of commonly used medical terminology and abbreviations, and VA acronyms and their meaning. Perform other duties as assigned. Work Schedule: Monday-Friday 7:30am-4:00pm Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/17/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Responsible for (1) protecting that information from unauthorized release or from loss, alteration, or unauthorized deletion, and (2) following applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set forth in the Medical center's Automated Information Systems Users Agreement which the employee signs; Manages information security within areas of responsibility to assure that computer system security and confidentiality of sensitive information is maintained, and that employee access to sensitive information is limited only to that which the employee requires to perform their duties; Knowledge of an extensive body of rules, procedures, or operations to perform a wide variety of interrelated or non-standard clerical work and resolve a wide range of problems; Knowledge of time management and possession of high-level problem solving and organization skills; Knowledgeable of current VA manuals, handbooks, memorandum, directives, and similar resources governing the administration of the H&CBCP. You will be rated on the following Competencies for this position: ClericalCommunicationsComputer SkillsCustomer Service Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work is sedentary. Typically, the employee sits comfortably to do the work. However, there may be some walking; standing; bending; carrying of items such as papers, books, small parts or driving an automobile, etc. At times, the position requires carrying supplies and equipment not to exceed 50 pounds. The incumbent is required to maintain a valid driver's license.
