Duties
Serve as a Clinical Pharmacist Practitioner facilitating pharmaceutical care services and direct patient care programs within assigned service(s)
to ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulated standards.
Maintain collaborative and comprehensive medication therapy management clinics and services in designated areas of practice, under a scope of practice.
Delivers direct care to patients though a variety of modalities and ensure documentation for encounters and clinical decisions in the electronic health records.
Ability to communicate effectively and professionally with patients, and other members of the healthcare team with the goal to educate, promote patient safety, and practice advancement.
Reviews electronic medical record to monitor for appropriateness of drug therapy, including evaluation for adverse drug reactions, drug-drug and drug food interactions,
disease due to iatrogenic cause, medication therapy duplication, or unnecessary polypharmacy.
Identifies high-risk patients and implements interventions to improve quality and safety.
Performs drug usage evaluations, quality assurance/ performance improvement activities as required.
Requirements
- This position requires regular contact with children under the age 18 and a favorable determination of suitability to do so.
- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.

- A Personnel Security Investigation is required.
- A pre-placement medical examination is required.
- This position is a Testing Designated Position (TDP) subject to a pre-employment drug screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
- Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
- Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
- Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
- This position may require the incumbent to work on call during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.

Basic Requirement for Pharmacist:
Degree: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Exemption: You are exempt from the basic requirement above if you are a current federal employee occupying a position in the 0660 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
Licensure: A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia.
Medical Requirement: Must be able to distinguish basic colors.
Experience Required: One year of professional pharmacy experience which includes providing clinical pharmaceutical care to patients; advising on medication therapy management; and training or educating health care professionals on the use of drugs and other pharmaceuticals. This definition of experience is typical of work performed at the next lower grade/level position in the federal service GS-12.
Education
