This position is organizationally aligned under the Geriatrics and Extended Care Service Line (Home and Community Care Programs, Home Telehealth) at the Jesse Brown VA Medical Center in Chicago, IL. The incumbent will serve as a Program Support Assistant to the Lead Care Coordinator and all staff of the Home Telehealth (HT) program. Duties are varied, non-routine, and consist of handling data and coordination for program patients.
Duties
Major Duties include:
Responsible for collecting, compiling, and/or tracking statistical information in support of program operations and to access status or outcomes in relation to established performance measures, goals, and objectives; receives and processes documents and reports in accordance with established procedures; uses software applications to draw information from a wide variety of sources in order to prepare reports and assist in the planning, review, and reporting of data/statistical results of program studies.
Implements nationally standardized protocols for incoming data, organizes and maintains patient data, and retrieves a variety of computerized data. Tracks and trends issues related to program effectiveness and role implementation as defined by the national protocols. Responsible for helping to assure all staff members are aware of current DSS codes for HT and names of clinics utilized by program. The incumbent utilizes statistical data to compile monthly workload reports for all program staff.
Maintains census and communicating changes/problems that might be encountered to appropriate HT staff. Participates in performance improvement activities related to the program quality improvement process. Responsible for providing routine or occasional data/reports related to program operations as requested by HT Lead, VISN Program Manager, or coverage.
Attains and maintain computer skills on a variety of software programs, e.g., MS Word, PowerPoint, and Excel; file management, VistA, CPRS, etc. and communication software MS Outlook. Accurately collects, enter, and manage data, including patient data and program data; and will enter required data into the VISN HT database in a timely fashion. Incumbent will act as consultant to program team members regarding administrative matters. Incumbent will operate software for Home Telehealth program as well as other databases for VISN office. Attains competencies in operation of the Home Telehealth equipment to assist in troubleshooting programs for patients, answering questions for patients/staff/ customers, and supporting the Home Telehealth team.
Responsible for recording, monitoring, and documenting the management of HT program budget, and will include effective utilization of resources and frequent fiscal coordination. Will be responsible for projecting/tracking the annual estimates of a variety of programmatic costs. Also responsible for compiling and submitting various budget reports to VISN office as requested as well as maintain accurate account of actual expenditures.
Responsible for establishing and maintaining inventory establishing setup, and first response to the vendor site. Will justify and request replacements for outdated equipment and gathering relevant information.
Responsible for maintaining calendar of appointments of patients, meetings, conference calls, and scheduled leave as well as scheduling meetings, preparing travel papers, scheduling conferences, and assisting the care coordinators with all administrative functions.
Responsible for the set up of Home Telehealth equipment in the home of veterans and instruct the veteran and/or the caregiver on how to use the equipment. This responsibility includes being able to troubleshoot problems with equipment by phone or home visits.
Perform other duties as assigned.
Work Schedule: Monday-Friday 7:30am-4:00pm
Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/15/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: protecting information from unauthorized release or from loss, alternation, or unauthorized deletion; manages information security within areas of responsibility to assure that computer system security and confidentiality of sensitive information is maintained; takes action as necessary to report and address violations of information security; handles conflict and problems in dealing with the customer constructively and appropriately; knowledge and proficiency of word processing and spreadsheet software; knowledge of correspondence procedures sufficient to screen material prior to distribution for suspense dates, and to establish controls to ensure that deadlines are met; knowledge of office procedures, filing systems, and various reference and handbooks commonly used by the staff to classify, search for, and dispose of materials; knowledge of scheduling, patient information menus and clinic processes; knowledge of complex electronic mail system to transmit, receive, and acknowledge messages.

Physical Requirements: The work is sedentary. Typically, the employee sits comfortably to do the work. However, there may be some walking; standing; bending; carrying of items such as papers, books, small parts or driving an automobile, etc. At times, the position requires carrying supplies and equipment not to exceed 50 pounds. The incumbent is required to maintain a valid driver's license.
