The Clinical Pharmacist is the primary professional pharmacy staff member responsible for the daily operations of the pharmacy medication management systems, to include ward stock, unit dose and outpatient prescription dispensing, investigational drugs, and ambulatory care pharmacy programs.

The Clinical Pharmacist will be involved in the independent professional practice of pharmacy in accordance with the guidance set forth in FHCC policies and procedures, VA regulations, DoD regulations, and state and federal laws governing the practice of pharmacy. The clinical pharmacist will be under the administrative supervision of the supervisor of the section assigned (Outpatient Pharmacy). The clinical pharmacist handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. These include, but are not limited to: Reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; Interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; Reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; Works with providers, nurses and other health care professionals to ensure optimal patient medication needs; Appropriately screens for community care consults and approvals while entering community care provider profiles and processing orders associated with community care; Investigates and provide good patient medication issue solutions; Maintains automation equipment to ensure accurate filling and dispensing; Fills and/or verifies pharmacy technician non sterile compounding orders; processes, fills and verifies scheduled and non-scheduled federal legend drugs and medical devices, over-the-counter products and supply items; Contacting providers as appropriate; Documenting recommendations and interventions; Providing refill extensions and partial medication supplies as appropriate; Taking health and medication histories; Performing medication reconciliation; Providing drug information; Will submit QA forms as needed locally or to CMOP; Assisting in formulary management including therapeutic substitutions, non-formulary reviews and medication usage evaluations; Documenting and assessing adverse drug events (ADEs); Assisting in medical emergencies; Providing emergency fills for patients as appropriate; Counsel patients on medications and related matters; Provides good customer service when communicating with health care workers and patients; Operate pharmacy software; providing oversight of technical staff in all aspects of medication distribution. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). Quality of Experience: Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. Part-time Experience: Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-11 to GS-11. Physical Requirements: Principle duties require repetitive use of hands and fingers (e.g., use of a computer keyboard, etc.). May require lifting and carrying light loads (up to 44Ibs.), including boxes and equipment; stooping or kneeling (e.g., to pick up items from the floor, remove and replace items on lower shelves, and to file documents in lower file drawers, etc.); sitting, walking, or standing for long periods of time (4-8 hours). Normal ranges of hearing and vision are required to perform work and communicate effectively; must have the visual ability to distinguish between basic colors and shades of color. An ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously is necessary. Employees must be physically capable of operating various types of equipment used on the job. Employees may be required to work at various pharmacy locations as assigned. Employees may be required to work mandatory overtime during times of extreme staffing shortages. Work is performed in patient care, office, pharmacy, and storage areas of the facility. Work areas are adequately heated and lighted. Potential exists for: exposure to infectious patients or materials; exposure to hazardous and toxic substances including but not limited to cytotoxic chemotherapy, solvents, grease, oils, and cleaning solutions; sticks or cuts from hypodermic needles, lancets or other sharp items; musculoskeletal injuries if proper lifting and carrying techniques are not used; unusual musculoskeletal fatigue due to prolonged standing, electronic data processing, etc.; eye strain.
