ORGANIZATIONAL LOCATION: This Nurse (Clinical Care Coordinator) Position is located at the Colorado River Service Unit, Moapa Health Clinic, in Moapa, Nevada. The primary responsibility is to implement and initiate standardized disease management and health prevention guidelines across the patient care continuum. Assess patient's needs individually and/or as a group with the care team. The Position is supervised by the Supervisory Clinical Nurse.

Responsible for setting up and monitoring patients enrolled in disease specific home monitoring and communicating clinical information of changes to clinical plan between patient and provider/care team.
Develops maintains patient registries based on disease, health condition and/or health prevention initiatives. Assists with triage of patients, including triaging telephone calls and walk-in patients. Prepares patients for tests, exams and treatments.
Coordinates appointments and procedures for patients who need further care or continuity of care at other facilities. Provides counseling and teaching to patient and families on preventive, curative and rehabilitative measures involved in health care.
Responsible for monitoring and oversight of patient panels, ensuring entire care team is working together to address panel management for chronic and preventative measures (including GPRA goals, medical home goals, clinical quality outcomes, etc.) Leads team huddles to focus on more daily patient panel management.
Lead Duties: Instructs and educates when appropriate other care team staff in specific task and job techniques and checks on work progress and reviews completed work for accuracy and completion. Resolves simple informal complaints and refers serious and repetitive complaints to Supervisor.

Qualifications

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree From Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above, many positions may require experience in a specialty area of nursing.
GS-10 Basic Requirements:
Possession of a diploma, associate degree or bachelor's degree in Nursing from a professional nursing program and at least one (1) full year of professional nursing experience (equivalent to the GS-9 grade level): Management of nursing care in outpatient primary care environments and/or case management, emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services which are consistent with patient-centered medical home principles. Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; Able to provide patient and family education pertinent to a wide range of conditions and health factors.
GS-11 Basic Requirements:
Possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program and at least one (1) full year of professional nursing experience (equivalent to the GS-9 or above): Care Management involving patient assessment, initiating, and maintaining individual multidisciplinary plan of care involving patients and their families to give optimal care. Ability to make decisions and implement them in complex health care situations. Extensive experience in nursing practice, service, and education; make independent judgements related to assignments, coverage and supervision; provide training and mentor nursing staff and provide input on evaluations. ; OR
Successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
