The CCICM Supervisory Social Work Program Coordinator has administrative supervision and/or direct clinical practice responsibilities for social work staff across multiple specialty departments including specialty programs such as Oncology clinic, Transplant program, Dialysis, Inpatient Medicine units, Surgery unit, Neurologic Disease program, Spinal Cord Injury program, Physical Medicine & Rehabilitation department, Emergency Department, facility Ethics program, Medical Intensive Care Unit.

The incumbent serves as a Subject Matter Expert and a Leader responsible for oversight of day-to-day operations, practice oversight and data management of Care Coordination Programs; Care Coordination and Integrated Case Management (CC&ICM) and supervises social workers in the specialty programs within VA ECHCS. The incumbent functions in supportive and consultative role to the Social Work Executive/Chief of Care Management and Social Work Service, who is responsible for approx. 260 multidisciplinary staff within the service across the whole health care system and in addition in charge of professional practice, functional statements, recruitment, credentialing and privileging and advocacy for 189 Social Workers in other services, ensuring effective operations for all areas covered Rocky Mountain Network Human Resources Title 38/Hybrid Title 38 Functional Statement 554-56856-O by the Social Work Discipline. Care Management and Social Work Service covers the entire VA Eastern Colorado Health Care System locations serving about 275,586 Veterans, as well as Active Duty personnel that are about to separate from DoD in Military Facilities across Colorado, Montana, Wyoming, Utah, Texas, Oklahoma, Louisiana and Arkansas. Care Management and Social Work Service includes multitude of programs such as Primary Care, Home Telehealth, Geriatric Extended Care, Home Based Primary Care, Contract Nursing Home, Veteran Directed Care, Adult Day Care, PACE, Medical Foster Home, Spinal Cord Injury, Guardianship, Hem/Onc, Renal, Amputee, Traumatic Brain Injury and Poly Trauma, Intimate Partner Violence, CLC, Advanced Care Planning, Caregiver Support Program, Transitional Case Management, Care Coordination and Integrated Case Management, and eScreening.
Serve as a CC&ICM for social work Subject Matter Expert to Social Work Executive/Chief of Care Management Social Work Service and support a total of about 500 Staff (260 Multidisciplinary Staff in Care Management and Social Work Service and approx.. 200 Social Workers in other Services across the health care system)
Direct CC&ICM oversight and responsibility over all Complex Specialty Case Management Programs: GEC, HUDVASH, HCHV, ICHMR, Home Telehealth, Geriatric and Extended Care, TCM, BHIP, HBPC, and Specialty or other programs as determined.
Manage CC&ICM and oversight/supervision of other specialty program day-to-day program operations, results and data management.
Provide direct supervision to social workers to include assigned social workers at the GS 9/11 and 12 grades.
Provide analysis and evaluation of program data and computerized programs to identify system wide trends and needs to enhance the quality of service across all programs within CC&ICM.
Develop and monitor all programs' compliance in CC&ICM with practice, standards and guidelines across clinical services and overall Social Work Discipline.
Develop policies, procedures, and/or practice guidelines for the programs and participating in facility, VISN and national program development of CC&ICM.
Incumbent supervises employees to accomplish the work of the organization unit. Exercises supervisor authorities and responsibilities involving work assignment and review, as well as the administrative and personnel management functions relative to the staff supervised.
This position supervises an organizational unit (s) and performs other related duties as assigned.
Supervisory:
The incumbent is responsible to the Deputy Chief Social Worker, Care Management and Social Work Service but practices independently, exercising initiative and independent judgment in the day-to-day activities. Supervision and consultation are of an administrative and professional nature; with the Deputy Chief providing guidelines to the general policy and direction of CMSWS program the incumbent coordinates. As needed, the incumbent initiates consultation with the Deputy Chief, Chief, as well as with other authoritative resources at the Medical Center, such as Regional Counsel, Business Services, Fiscal, and Human Resource Management Services

Work Schedule: Tours between 07:30am and 4:00pm depending on supervisory needs

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/02/2024.
Basic Requirements:
United States Citizenship.
Education. Possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. For grades below or at the full performance level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent master's level within 3 years of their appointment as a social worker.
Licensure. Possess a full and unrestricted license or certification by a state to independently practice social work at the master's degree level.
English Language Proficiency. Social workers must be proficient in spoken and written English.
Grade Determinations:
Social Worker (Supervisor), GS-13
Experience and Education. One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills in administration, demonstrating progressively more professional competency skills and judgment. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as supervisory social workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Ability to independently organize work, set priorities, and meet multiple deadlines.
Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
Ability to ensure provision of clinical social work services by supervised social workers.
Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion.
Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service.
Ability to provide staff training and development.

Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Social Worker must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience and control emotions with reasonable accommodation, if necessary, without endangering the health and safety of the Social Worker or others. Social Worker must be able to pass VA requirements for operation of a government vehicle. Position requires walking throughout the clinic and/or Medical Center, standing for extended periods of time and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams.
Position may require some travel to meet with VISN, VACO, other VA Facilities, or community agencies and partners who aid and support Social Work Service, its various programs, and Veterans served.
This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
Preferred Experience:
Preferred experience with Care Coordination and Integrated Case Management (CCICM) including operational knowledge and process improvement.
