Duties
Serve as the point of contact and team lead; works closely with provider, nursing, and administrative team to ensure smooth/seamless care within the medical home.
Perform independent, comprehensive telephone and clinical triaging, and facilitates care coordination through a multidisciplinary approach; establishing priorities for patient care and evaluates patients’ progress toward high-level wellness.
Provide patient education, acts as relief nurse, triages and screens walk-in patients to assess the severity of the patient's condition and the need for provider intervention; communicates with providers regarding determinations and situations
Screen patient charts to assess the medical history and determine health care needs; makes psychosocial assessment of patients to identify significant cultural practices, the ability to understand and cooperate, and expectations of care.
Execute or ensure the completion of orders from health care providers; recognizes and reports abnormal test results, changes in symptoms and the need for follow-up; records data on patient charts.
Assist health care providers with examinations and procedures; oversees educating patients for high risk, painful or stressful procedures with education and support.
Prepare and administer oral, subcutaneous, intra-muscular and intravenous medications.
Assist with patient and staff education; assists with clinic orientation, and one-on-one patient, educational activities related to acute or chronic illnesses, self-care, wellness/preventive health measures, and normal development and physiology.
Develop appropriate nursing plans; follow quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions.
Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network; assist patient to comply with medical treatment plan as necessary through telephone contact or through clinic visits.

- A Personnel Security Investigation is required.
- Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
- This position is a Testing Designated Position subject to pre-employment screening and random drug testing. Selectees will be required to consent to participation in random drug urinalysis testing.
- A pre-placement medical examination is required.
- Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
- Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
- Applicants must be able to fulfill non-privileged professional credential requirements.
- This position may require the incumbent to work during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.

Basic Requirement for Nurse (Clinical/Ambulatory):
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.).
Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: Graduates of foreign nursing schools must have completed the Commission on Graduates of Foreign Nursing Schools Certification Program. The certification program consists of a credentials evaluation, the CGFNS Qualifying Exam, and evidence of passing one of the approved English Language Proficiency examinations. A copy of the CGFNS Certificate is required.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes providing ambulatory nursing care to patients in a Patient Centered Medical Home setting; administering medications and providing education to patients and/or patient families.
OR
Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
Exemption: You are exempt from the qualification requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.
