Serves as LPN and is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The LPN demonstrates active collaboration, communication and problem solving with the surgical team. The LPN is responsible for actively participating as part of a high functioning team in meeting the surgery department initiatives and outcomes.
Duties
The incumbent serves as a Licensed Practical Nurse (LPN) at the point of care for the Kansas City VA Medical Center, Outpatient, and its Community Based Outpatient Clinics. The LPN function is to provide quality nursing care to a culturally diverse veteran population of varying complexity in a variety of settings - primary care, acute care, sub-acute, rehabilitation, spinal cord injury, palliative and long term care, inpatient, outpatient, community care, and home care.
The practice of each LPN is based on skill, experience, and research and has a direct impact on client outcomes for the assigned area and the facility as a whole.
The LPN possesses the knowledge and skills necessary to effectively apply aspects of the nursing process within a collaborative, interdisciplinary practice setting. This broad based clinical knowledge enables the LPN to effectively implement health promotion and prevention interventions, manage acute and chronic illness states, assist veterans in the attainment of optimal levels of functioning through rehabilitation, and provide supportive measures for the dying patient and their significant others. The LPN possesses the knowledge and skills necessary to:
- Communicate and interact appropriately and courteously with internal and external customers.
- Maintain confidentiality of patient / employee information electronically, verbally, and written.
- Work and function within a safe manner including infection control and operation of equipment.
- Integrate and apply pertinent aspects of the Medical Center performance improvement program into his/her professional practice.
- Utilize nursing research, evidence based practice and literature in daily practice.
- Provide care appropriate to the bio-psychosocial needs of the population served. This knowledge includes an understanding of the changes associated with aging and principles of growth and development relevant to the adult and geriatric population. He/ she must be able to assess and interpret data relating to the patient's current health status, which will facilitate the delivery of care appropriate to the age specific needs of the adult and geriatric patient.
- Provide culturally competent care to a diverse patient population.
- Practice requirements include BLS and/or ACLS where appropriate.
Work Schedule: M-F 7:00 am - 3:30 pm

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by ACEN or CCNE at the time the program was completed by the applicant. Verification can be obtained from the State approving agency, ACEN, or CCNE. Exceptions include:
Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions
Individuals granted a license by a jurisdiction that does not require graduation from an approved school.
Licensure: Full, active, current and unrestricted licensure as a [ ] licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).Grade Determinations: GS-7: The GS-7 grade level is for select, complex LPN/LVN positions, as established by each health care facility. To be advanced, employees must meet all of the following standards, provided the standard is part of their assignment:
Experience: Completion of at least one (1) year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all of the performance requirements for the GS-6 LPN/LVN.
Knowledge and skills necessary to carry out more specialized patient care duties, including assignments in selected, more complex inpatient and/or outpatient care settings. Knowledge base promotes the incorporation of current patient care data in actively contributing to the development of the patient's overall health plan and in carrying out assigned nursing interventions. Positions at this grade level require broader knowledge and skills because supervision is more general and employees are expected to be involved to a greater degree in the development and modification of treatment plans.
Preferred Experience in Surgery Clinic and Surgery care coordination, but not required.
