This Pharmacist (Clinical) is located at the Phoenix Indian Medical Center, Phoenix, AZ. The hospital is a general, acute care hospital with an authorized capacity of 127 beds. The incumbent independently provides comprehensive range of clinical pharmacy service; serves as a consultant, provides clinical teaching activities, and assists with performance improvement initiatives. The primary duties are performed in the inpatient setting.

Duties

Serves as authoritative consultant on known and potential effects of drugs, those drugs recently approved for use, those in an investigational and/or clinical evaluation status, or other drugs which do not yet have approval for general use. Serves as coordinator on evening, night, and weekend shifts, organizing and supervising pharmacists, pharmacy technicians and other support personnel; advises the medical and nursing staff concerning such matters as the clinical effects of drugs, proper dosage, general administration, toxicity, precautions, alternative medications, contraindications, adverse reactions, synergistic actions and dosage forms; reconciles medication lists and counsels patients, family members, and/or care-takers on drug therapy. Collaborates with other health professionals to develop treatment protocols, standards of practice and policies. Participates in ongoing quality assurance and performance improvement activities including audits, chart reviews, development of review criteria. Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. BASIC REQUIREMENT(S): All pharmacists must meet all three of the following basic requirements: Degree: A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Licensure: All applicants must have passed the North Pharmacist Licensure (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE). Applicants also must be license to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Medical Requirement: Must be able to distinguish basic colors. Experience: Applicant's qualifications background must demonstrate the knowledge, skills, abilities, and competencies necessary to perform the work of the position. Pharmacy work requires knowledge of the use, clinical effects, and composition of medications, including their chemical, biological, and physical properties. Qualifying professional pharmacy experience may involve, but is not limited to: Dispensing medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects; Evaluating medication use patterns and outcomes for patients in hospitals or managed care organizations; Performing administrative, consultative, or staff advisory work for a medical facility's pharmacy program; Planning, monitoring and evaluating medication programs or regimens; Establishing medication-handling procedures for the storage and preservation of medications; Researching medical literature and/or clinical medication information to provide accurate responses to inquiries; and/or Maintaining all medication records required by law. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below: MINIMUM QUALIFICATIONS, GS-12: In addition to the requirements for the GS-11 (or equivalent) level AND your resume must demonstrate at least one (1) year of specialized experience equivalent to the GS-11 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Independently collaborates with physicians in the design of drug treatment plans; monitors drug therapy to assure appropriateness; modifies therapy when required; evaluates drug therapy for proper therapeutic endpoint; designs drug regimens utilizing current pharmacokinetic principles and specific patient parameters; performs daily evaluation of admitted patients to monitor and review drug therapy for rational indications, dosing, adverse drug reactions, drug interactions, IV incompatibilities and complications of multiple drug therapies; monitors adverse drug reactions and reports all significant findings as required; serves as pharmacy liaison to other departmental staff to resolve drug related problems. Selective Placement Factor: In addition to the basic requirements and minimum qualifications, the following has been determined to be essential to perform the duties of this position: Board Certified Pharmacotherapy Specialty (BCPS) and have at least three (3) years of experience providing pharmaceutical care in a hospital which included critical care activities or have completed a Pharmacy Practice Residency followed by at least one year of experience providing pharmaceutical care in a hospital which included critical care activities. You must meet all qualification requirements by the closing date of the announcement.
