Two positions are located in Social Service Department in Whiteriver Service Unit at Social Services Department and is located in Whiteriver, AZ. The incumbent provides a broad range of direct social work services to individuals and families. Periodic and scheduled consultation with the supervisor as appropriate for guidance and the evaluation of efficiencies, challenges, and progress in the delivery of care, treatment, and services. This position reports to the Social Services Director.
Duties
Responsible for providing a full range of social work services to patients and/or their family members. Services include but are not limited to the following: Screening assigned inpatient, outpatient, and community referrals to determine psychosocial needs of clients, patients and their families, and for the arrangement of follow-on care, treatment, and services. Incumbent will provide information on available community resources to patients and/or family members, e.g., intimate partner/family violence shelters, housing, home health care and hospice programs, etc.
Participates in hospital discharge planning, care coordination, and teaching conferences with health care providers.
Provide social service consultations to various hospitals, clinical staff and have rudimentary knowledge of age specific interventions.
Coordinates closely with medical and nursing staff in resolving challenges related to discharge planning. Incumbent will clarify patients' understanding of health care and social service care, treatment, and services and will develop options for continuing care.
Maintain accurate and timely records of all patient encounters in accordance with applicable policies, procedures, and Joint Commission standards.
Qualifications

BASIC REQUIREMENT(S):
Completed a Master's degree, or higher doctoral degree, in social work from a school accredited by the Council on Social Work Education.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS, GS-0185-11:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-09 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Examples include: performs intensive and wide range of behavioral health services including moderate technical difficulty including evaluation, consultation, counseling, referral, psychotherapy, crisis intervention and treatment of mental, emotional and behavioral disorders to patients and families.
OR
Completion of 3 years (54 semester hours) of graduate level education leading to a Ph.D. degree or Ph.D. that is directly related to the work of the position.
OR
Possess a combination of education and experience, in one or more of the behavioral or social sciences equivalent to a major in the field, that when combined fully meet the minimum qualifications for this position. The total percentage equals at least 100 percent to qualify for this GS-11 grade level. Note: Only graduate level education in excess of the amount required for the next lower grade level (2 years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree) may be combined with experience.
MINIMUM QUALIFICATIONS, GS-0185-12:

Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
