James A. Haley Veterans Hospital is looking for Social Worker (Medical Foster Home Program Coordinator). This position reports under Tampa Medical Center with Home Base visits mainly in Citrus and Hernando Counties. However, visits to Pasco, Hillsborough and Polk counties might be required if coverage needs it. The Social Worker assesses the needs of high-risk patients and provides care management services within the interdisciplinary team.

The Social Worker (Medical Foster Home Program Coordinator) works with an interdisciplinary team and provides consultation services to the team and other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan. They manage and control the use of data related to these programs regarding day-to-day functioning and program evaluation.
Additional duties include but not limited to:
Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to patients.
Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction.
Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
Will develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others, whenever possible. The assessment will include a home visit.
Working within the interdisciplinary treatment team to develop a treatment plan with the veteran based on this assessment.
Initial assessment will be completed within 14 calendar days.
Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team.
Possesses the skills to offer psychosocial interventions and measures outcomes to interventions.
Attends weekly interdisciplinary care planning meetings.
Ongoing assessments and updating treatment plans are done when necessary or on a quarterly basis given the changing needs as health status changes.
Address the unique needs of homebound, frail, disabling/chronic and/or dying veteran.
Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
Independently identifies high-risk patients and provides case management services.
Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members.
Has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
Establishes and maintains effective therapeutic relationships with veterans and their families.
Independently work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills.
Possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups.
Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed.

Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications
Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Social Worker (Program Coordinator) candidates must have the following:
Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.

Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive a government car.
Preferred Experience:
* Program Coordination and Case management experience is preferred
* Medical and mental health clinical experience needed for completion of bio-psychosocial assessments, implementation of
treatment interventions, and determination of appropriate level of care.
* Knowledge of community resources
* Crisis intervention skills and the ability to remain calm in high pressured situations is mandatory to effectively problem solve and implement effective clinical interventions.
The full performance level of this vacancy is GS-12.
