James A. Haley Veterans Hospital is looking for Social Worker (Medical Foster Home Program Coordinator). This position reports under Tampa Medical Center with Home Base visits mainly in Citrus and Hernando Counties. However, visits to Pasco, Hillsborough and Polk counties might be required if coverage needs it. The Social Worker assesses the needs of high-risk patients and provides care management services within the interdisciplinary team.

They manage and control the use of data related to these programs regarding day-to-day functioning and program evaluation. Additional duties include but not limited to: Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Will develop an assessment of the veteran in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others, whenever possible. The assessment will include a home visit. Working within the interdisciplinary treatment team to develop a treatment plan with the veteran based on this assessment. Initial assessment will be completed within 14 calendar days. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Possesses the skills to offer psychosocial interventions and measures outcomes to interventions. Attends weekly interdisciplinary care planning meetings. Ongoing assessments and updating treatment plans are done when necessary or on a quarterly basis given the changing needs as health status changes. Address the unique needs of homebound, frail, disabling/chronic and/or dying veteran. Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Independently identifies high-risk patients and provides case management services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Establishes and maintains effective therapeutic relationships with veterans and their families. Independently work with patients and families who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group and family counseling and therapy skills. Possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups. Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker (Program Coordinator) candidates must have the following: Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
