This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Primary responsibilities are to provide clinical evidence-based services and interventions; program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality-improvement activities and evaluation/consultation. The Coordinator will function in support of caregivers, Veterans and family members throughout the Medical Center and in the community and will support the development and implementation of General Caregiver services. This includes development of a strategic plan for expansion of the General Caregiver Support Program and conducting frequent needs assessments to implement education, wellness, and support groups. The Coordinator is administratively responsible and accountable for the clinical and program development and effectiveness of the facility General Caregiver Support Program. The Coordinator participates in direct caregiver and Veteran communication to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions. Advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues. Creates educational tools, develops programs and implements training focused on specific caregiver needs/issues. Provides education and in-service training to VAMC employees, Veterans, families and caregivers. Provides ongoing consultation and support to colleagues regarding caregiver support. Develops and facilitates caregiver support groups (face-to-face or VVC) and educational programs. Distributes educational materials provided by VA, local, state and national caregiver organizations. The Coordinator serves as a liaison between the VA and community agencies. Participates in caregiver support activities in the local community and state to promote and recognize the role of the caregiver. Participates on state and local caregiver support committees. Invites state and community caregiver related agencies to participate on the local VA Caregiver Support Advisory Board. Establishes and maintains access to electronic social networks such as Facebook, Twitter, secure email and YouTube in accordance with VA policy. Develops and fosters relationships with community partners to assist with system navigation, maintain collaboration and coordinate services. Provides outreach presentations to community social service agencies, military Guard and Reserve units, Veteran Service organizations and other groups on VA caregiver benefits, programs and services. Provides consultation, guidance and expertise to community organizations regarding Veteran caregiver specific issues and needs. Promotes the VA Caregiver Support Program in coordination with the local VA Public Affairs Officer through interviews with local print, voice and television media. Maintains basic knowledge of local VA medical center programs, community services and VA Central Office caregiver support initiatives. Maintains knowledge of current federal and state legislation related to caregiver initiatives. The Coordinator will develop a library of caregiver support information materials for distribution through the various inpatient/outpatient and specialty clinic settings. Educational materials must be specific to the age group serviced in the clinic, i.e. Alzheimer's disease and Parkinson disease in the general and geriatric settings and PTSD and TBI information in the general, OEF/OIF, post-deployment and Polytrauma Center settings. Uses knowledge of the CSP to matrix veterans level functioning with the appropriate CSP services (ie. PCAFC and PGCSS) to ensure patient safety recovery program stewardship and organizing the delivery of care to assure continuity of care and peer accountability for practice including access to care and discharge planning. Applies advanced clinical knowledge judgment to promote staff involving in planning decision making and evaluating PCAFC (Program of Comprehensive Assistance for Family Caregivers) and/or PGCSS (Program of General Caregiver Support Services) eligibility. Receives, reviews and dispositions PCAFC and/or PGCSS initial eligibility decisions reassessments and appeal decisions for clinical eligibility determination The Coordinator coordinates the events and programs for the annual National Family Caregivers Month every November that includes announcement and advertising of month-long activities, mobilization of participants, educational sessions, caregiver recognition events and caregiver support programs. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Experience: At least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment and are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
