This position is located at the Armed Forces Retirement Home (AFRH) in Health Care Services and performs clinical work independently in providing a range of social work services that are essential to the wellbeing of Residents/families in Long-Term Care and Independent Living. The incumbent is responsible for providing admission preparation, assessment, counseling, day to day assistance, locating and arranging resources, internal and external advocacy, education, and discharge planning..
Duties
As a/an Social Worker, you will:
Provide social work services for a geriatric population who experience a variety of complex social, medical, and psychiatric problems.
Ensure that Residents' self-determination is safe guarded to the extent possible through actively involving them in planning and decision-making.
Render advice and handles the Resident's personal financial aspects by setting up controlled accounts and obtaining coupons.
Complete admission preparation.
Be responsible for the completion and updating of the living will and advanced directives.
Qualifications
You must meet the following requirements by the closing date of this announcement.
Specialized experience: For the GS-11, you must have one year of specialized experience at a level of difficulty and responsibility equivalent to the GS-09 grade level in the Federal service. Specialized experience for this position includes:
Working with a geriatric population where the incumbent was responsible for providing admission preparation, assessment, counseling, day-to-day assistance initiation and revision of care plans and integrate services to effectively coordinate resident/patient care.
OR
You may substitute education for specialized experience as follows:
3 years of progressively higher level graduate education leading to a Ph.D. degree or Ph.D. or equivalent doctoral degree.
AND
Licensure: Must possess a state issued Social Worker's license
AND
A Social Worker's license to practice as an independent licensed practitioner.
AND
Education: Must possess a master's degree in social work from a school accredited by the Council on Social Work Education.
