This position is located at the Anadarko Indian Health Center under the supervision of the Supervisory Nurse or designee. As the RN-Care Manager (RN-CM), the incumbent is responsible for all aspects of Care Management for empaneled patients assigned to their designated care team, assisting in the development of the Plan of Care, ensuring that the plan is implemented in a timely basis, and identifying patient needs. The incumbent provides care management in the ambulatory health care setting.
Duties
Collaborate with primary care providers, clinic nurses, public health nursing, DM case management and social services to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit.
Serves as primary contact/clinical liaison between referral hospitals, treatment facilities, and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established plan of care.
Contacts patients for follow-up diagnostic tests, referrals, appointments, or other medical issues as appropriate in collaboration with the provider.
Interacts with the patient/family/caregiver and primary care provider to develop and discuss the patient's plan of care, understanding of disease processes, and coordination of services based on patient's clinical needs and available resources.
Conducts intake assessment, screening, and follow-up interviews with client and/or family members and/or partner(s) at office, by telephone, in the clinic, or at the home of the client.

BASIC REQUIREMENT(S):
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education , or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below.
MINIMUM QUALIFICATIONS:
One (1) full year of professional nursing experience (equivalent to the GS-9 grade level) AND possess a diploma, associate degree, or bachelor's degree, or master's degree from a professional nursing educational program. Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: is defined as experience in performing comprehensive clinical assessment skills in the adult population and chronic disease management in addition to serving as a consistent member of a multidisciplinary team whose purpose is to conduct care coordination for patients beyond what is offered in the immediate facility (i.e. home health care, hospice care, rehabilitation therapy, skilled nursing facilities, specialty services, durable medical equipment (DMEs), tribal resources, etc.)
OR
Possess a Ph.D. or equivalent doctoral degree from a professional nursing educational program or related medical science field.
OR
One (1) full year of professional nursing experience (equivalent to the GS-9 grade level) AND possess a diploma, associate degree, or bachelor's degree, or master's degree from a professional nursing educational program.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
