The Organizational Location of the Position is: Program IV, in Physical Medicine and Rehabilitation Service, VA North Texas Health Care System, Bonham Outpatient Clinic. Work is mainly Monday through Friday with rotations for weekends and holidays for patient care coverage.
The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-09 to GS-12.
Duties
Duties include but not limited to:
Patient Care:
Veterans are scheduled for evaluation considering caseload complexity and acuity of medical condition. Coordinates Veteran schedule with other interdisciplinary team members.
Recognizes the appropriateness ·of treatment based on indications, precautions and contraindications (e.g. pharmacological) and uses professional judgement to initiate consultation to the appropriate provider.
Evaluations reflect the complexity through Occupational profile and client history; Assessments of occupational performance; Clinical decision-making and development of a patient centered plan of care.
Evaluations include medical history, results and interpretation of relevant evidence-based methods and appropriate functional tests and measurements such as (but not limited to): range of motion, muscle strength, sensory/perceptual deficits, cognitive deficits, functional AOL's, pain, endurance, neurological deficits, personal and psychosocial factors relating to the specific rehabilitation program they serve.
Demonstrates the skilled ability to evaluate client factors (specific capabilities, characteristics or beliefs), performance skills (motor skills, processing skills and social interaction), performance patterns (habits, routines and roles), environment (physical & social), and context (cultural, personal, temporal and virtual), the therapist in this position demonstrates the ability to analyze Veterans occupational performance in order to target identified outcomes.
Assess the patient's medical conditions, functional capabilities, limitations, restrictions and rehabilitation potential.
Establishes a care plan and administers a treatment program with specific goals according to the Veteran's capacity and tolerance.
Treatment Implementation:
Evaluation findings are used to develop and implement comprehensive patient treatment plans including objective, measurable goals and timeliness for completion.
Implements complex and innovative therapeutic techniques not limited to: Therapeutic media, lifestyle coping skills and modifications, complementary and alternative modalities (CAM), cognitive therapy and mobility training to maintain or achieve veteran's physical and/or emotional rehabilitation potential.
Independently develops and implements comprehensive and advanced therapeutic programs and techniques.
Demonstrates knowledge of occupational therapy principles and techniques consistent with current clinical standards based on OT theory and evidence based practice.
Recognizes and responds to adverse reactions and emergency situations of the patient during the session and modifies the plan of care. Documents treatment progression and/or modifications accordingly.
Provides the equipment, materials, instruction, encouragement, or counseling necessary to promote functional mobility and activities performance.
Documents patient care services timely with 24 hours. Documents care in CPRS, completes encounter, FIM and other outcomes in accordance with PM&R, CMS and VANTHCS policies.
Appropriately plans Veteran discharge and/or needs for continuing treatment and terminates treatment when the Veteran reaches maximum rehabilitation potential.
Develops Home Programs for patients as indicated at discharge. Instructs significant others in ways to assist patient to carry out home program in the use of equipment and adaptive/assistive devices issued.
Administration Functions:
Identifies, collects and analyzes appropriate patient care information to provide a data base to develop performance evaluation system(s) and Process Improvement projects related to the clinical setting.
Participates in update of evaluation forms and protocols to adequately meet the requirements of the program and types of patients in assigned clinical setting.
Delegates tasks/procedures to the appropriate support personnel to facilitate effective and efficient patient care.
Ensures patient privacy and confidentiality by following established policies and procedures (Privacy Act, Release of Information, HIPPA, etc.).
Seeks clarification, listens and conveys information clearly and effectively, in written and verbal forms.
Demonstrates the ability to identify and resolve interpersonal or professional conflicts when dealing with patients and members of the organization.
Participates in ward(s) rounds, huddles, IDT, T JC/CARF/OIG activities, and any other relevant activities as needed.
Assists in ordering adequate supplies and maintaining inventory for assigned clinic(s).
Develops safe work habits to prevent injury to veterans, peers and self.
Performs other related duties as assigned.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
HPTs who are on track to graduate/complete the requirements should respond as if they have met the requirements. Provide details in the narrative sections of the assessment.
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education and/or Experience. See Below
1. The individual must meet at least one of the following requirements below:
Bachelor's degree in occupational therapy and two (2) years of experience as an occupational therapist;
NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that the Accreditation Council for Occupational Therapy Education (ACOTE) would only accredit master or doctoral degree programs in occupational therapy.
Bachelor's degree in occupational therapy and two (2) full years of graduate education in a related field;
NOTE: The baccalaureate degree must be from an approved program prior to the AOTA January 1, 2005 decision that ACOTE would only accredit master or doctoral degree programs in occupational therapy.
Master's Degree or higher in occupational therapy.
2. Individuals must be a graduate of a degree program in occupational therapy approved by the ACOTE or predecessor organizations. This is inclusive of an internship (supervised fieldwork experience required by the educational institution).
Foreign Graduates. Graduates of foreign occupational therapy programs meet the requirements when possessing a full, current, and unrestricted state license, to practice occupational therapy in a state, territory or Commonwealth of the United States, or in the District of Columbia.
Certification. Candidates must possess a current NBCOT certification as an OT.
State Licensure. Candidates must possess a full, current, and unrestricted state license, to practice occupational therapy in a state, territory or Commonwealth of the United States (i.e., Puerto Rico), or in the District of Columbia.
Grade Determinations:
GS-9
Education, Experience, or Licensure: None beyond the basic requirements.
GS-11
Education, Experience, or Licensure:
Completion of one year of experience equivalent to at least the GS-9 grade level and directly related to the position being filled; OR
Three years of progressively higher level graduate education leading to a degree in occupational therapy or a directly related field; OR
Doctorate in occupational therapy.
Demonstrated Knowledge, Skills, and Abilities (KSAs): In addition to the [experience or education above, the candidate] must demonstrate all of the following KSAs:
Knowledge of occupational therapy practice.
Ability to administer/interpret evaluation findings to develop and coordinate intervention plans, including goals and methods of treatment.
Ability to implement intervention plans directly or in collaboration with others.
Skill in monitoring an individual's response to interventions and modify treatment plans and reevaluating as indicated.
Ability to communicate and or collaborate with patients, family members, caregivers, interdisciplinary professionals and/or other individuals verbally and in writing.
Knowledge of health and safety regulations to minimize risk in the provision of patient care and the environment of care.
Knowledge of applicable regulations governing documentation, reimbursement and workload entry in accordance with established professional practice.
GS-12:
Education, Experience, and Licensure: Completion of one year of experience equivalent to at the GS-11 grade level and directly related to the position being filled.
Demonstrated KSAs: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of occupational therapy principles and techniques consistent with current clinical standards based on OT theory and evidence based practice. Knowledge is inclusive of physical, occupational, cognitive, and psychosocial functional deficits.
Ability to collaborate and communicate orally and in writing with all internal and external stakeholders.
Ability to use critical analysis, clinical reasoning, and creativity to independently solve complex problems related to adapting and modifying assessments, treatment plans, activities and procedures to meet the needs of patients.
Skill in procuring, fabricating, adjusting, adapting, and modifying orthoses, splints, and adaptive equipment for activities of daily living (inclusive of durable medical equipment)
Ability to conduct OT related in-service and clinical training.
Preferred Experience:
Experience in general and complex evaluations with skills/knowledge to establish treatment plans for multiple diagnosis.
Independently develops and implements comprehensive and advance therapeutic programs and techniques.
Ability In ordering adequate supplies and maintaining inventory/equipment for assigned clinic.
Experience in Power Mobility/wheelchair evaluations and fitting for Direct Access

Exceptions for the Graduate Occupational Therapist.
OT graduates from an approved occupational therapy program [who otherwise meet the minimum qualification requirements,] but who do not possess NBCOT certification and/or state [licensure], may be [appointed, pending certification and/or licensure,] as a graduate OT [on a full-time temporary appointment] not-to-exceed two years under the authority of 38 U.S.C. § 7405(c)(2).
Graduate OTs may only be appointed at the GS-9 grade level and may not be promoted/converted to the GS-11 level until licensure and/or certification is obtained. For grades levels at or above the developmental GS-11 grade level, the OT must be certified and licensed.
A graduate OT may provide care only under the direct supervision of a licensed OT who meets all state regulatory requirements
Temporary graduate OT appointments may not be extended beyond two years, or converted to a new temporary appointment.
Grandfathering Provision. All persons employed in VHA in this occupational series or in another occupational series that are also performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/certification/registration that are part of the basic requirements of this occupation.
