This position is located at the Armed Forces Retirement Home- Gulfport (AFRH-G) in Health Care Services. The Social Worker performs clinical work independently in providing a wide range of challenging social work services that are essential to the wellbeing of Residents/families in Long-Term Care (LTC) and Independent Living.

Duties

As a Social Worker, you will: Ensure that Residents' self-determination is safe guarded to the extent possible through actively involving them in planning and decision-making. Locates and arranges resources, understanding how systems interact and what resources are available to meet the identified needs. Effectively relates to the Resident's family members who are feeling guilt related to placement. Using expert knowledge, manages Residents with palliative care and End-of-life issues both with the Residents and family members. Is responsible for the completion and updating of the living will and advanced directives. Be responsible for setting up telephone, cable television, and laundry services. Additionally is involved with the coordination of repair for telephone and cable services.

Qualifications

You must meet the following requirements by the closing date of this announcement. Specialized Experience for the GS-11: You must have one year of specialized experience at a level of difficulty and responsibility equivalent to the GS-9 grade level in the Federal service. Specialized experience for this position includes: Working with a geriatric population where the incumbent was responsible for providing admission preparation, assessment, counseling, day-to-day assistance initiation and revision of care plans and integrate services to effectively coordinate resident/patient care. OR You may substitute education for general or specialized experience as follows: 3 years of progressively higher level graduate education leading to a Ph.D. degree or Ph.D. or equivalent doctoral degree. OR You may qualify by a combination of experience and education. Options for qualifying based on a combination will be identified in the online questions. SPECIAL NOTE: Long-term care experience and/or interacting with geriatric patients is highly desired.
