This position is responsible for nursing care management services at the Colville Service Unit, Purchase/Referred Care (PRC) program in Nespelem, WA. The primary purpose will be to provide case management, follow-up of continuity, and coordination of care for patients of all ages and those sent to other facilities on Contract funding/Purchase Referred Care. https://www.ihs.gov/portland/healthcarefacilities/colville/

Duties

Conduct initial and ongoing health care screening and assessment as part of the registration process and updates to patient files. Conduct planning for scheduled patients and communicate the patient needs and recommended treatment to care team members. Maintain liaison with other departments, agencies, organizations, and community resources to determine appropriate utilization. Assist with reviewing records and making appropriate referrals, coordinating care and integrating clinical services, coordinating the case management and discharge planning processes, and determining available third-party resources through monitoring patient admissions and transfers. Performs comprehensive professional nursing duties. Works closely and communicates with providers, nurses, social services and other disciplinary teams to update on patient care. Arrange specialty appointments and procedures for patients at other facilities, Develops and maintains disease specific registries for various patient with chronic illnesses. Assists with ordering equipment and needed supplies for patients. Organizes patient information, such as referral medical care documents, appropriate lab or x-ray information, and documents of Public Health Nursing referrals or home health visits. Review medical information to determine the best care plan with collaboration with a multidisciplinary team. Provide health education to patients with targeted illnesses through brochures and direct teaching. Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S):Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing. A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education , Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing. Basic Requirements for the GS-11 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: GS-11: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Developing a tracking system for patient care coordination and care management across the continuum; independently planning nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; experience with a wide variety of medical disorders and understands the normal course of disease with anticipated complications and indicated therapeutic interventions, providing care management for patients by coordinating services to meet patient's care goals; ability to communicate effectively with a wide variety of people, including patients, staff or other facilities, nurses, doctors; providing oversight to ensure compliance and appropriateness of care. You must meet all qualification requirements within 30 days of the closing date of the announcement.
