The New Mexico VA Healthcare Network is recruiting for a Social Worker (Homeless Patient Aligned Care Team (HPACT) Outpatient Coordinator). The incumbent will work directly with internal and external stake holders assisting the coordination of homeless program resources across the VA and community homeless Continuum of Care. The incumbent will also provide case management for veterans assigned to the HPACT.
Duties
Duties include but are not limited to:
Assist with Homeless Hotline calls.
Provide assessment and screening required for Veterans to access the community respite program resources.
Engage with community partners, developing networks of appropriate referral resources, and facilitating and coordinating access to needed services.
Manage the day-to-day processes specific to communication and coordination with the Homeless Respite Contractor.
Conduct clinical needs assessments, care planning, case management, and follow-up support with Veterans experiencing or at risk for homelessness.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Licensure: Candidate must be licensed or certified by a state at the advanced practice level, and must be able to provide supervision for licensure.
Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-12 Social Worker (Program Coordinator) Experience: In addition to the basic requirements, applicants must have at least one year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
In addition to the experience above, candidates must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Preferred Experience: Experience working with Veterans who are experiencing, or are at risk of homelessness. Demonstrate experience working with community partners to identify and access housing supports and other psychosocial resources. Experience conducting outreach to vulnerable populations, and developing community resource networks.

The full performance level of this vacancy is GS-12.
