The Social Services Assistant is an active member of the Housing and Urban Development-VA Supportive Housing Program (HUD-VASH). You will assist Veterans through the provision of concrete services such as housing location, transportation and community referrals, as well as providing consumer information and support for veterans in outpatient and inpatient settings. Duty Locations: Topeka, KS & Leavenworth, KS.
Duties

Duties include, but are not limited to the following:
Assists customer/patient in conjunction with treatment team to complete steps to achieve housing and community living stabilization;
Assists in providing and coordination of concrete services including transportation, food, referrals to community agency, financial planning;
Assists in obtaining housing by taking them to view housing either meeting them or arranging/providing transportation;
Supports healthy housing choices around leisure, vocation and lifestyle and matching strengths and preferences to these choices;
Assists in building social skills in the community that will enhance housing and community stabilization;
Serve as a recovery agent by providing and advocating for any effective services to aid in daily living;
Assists in obtaining services by referral and connection to community resources or groups to support community stabilization;
In conjunction with the HUDVASH case manager, will work to develop a treatment plan based on strengths, needs, abilities and goals;
Document all client interventions in the electronic medical record following accreditation and local standards. All notes co-signed by the HUDVASH case manager or designee;
Performs other related duties as assigned.
Work Schedule: Full Time, Monday - Friday 8:00am - 4:30pm

Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. Qualifications
To qualify for this position, you must meet one of the following:
Specialized Experience: Requires one (1) full year of specialized experience that equipped you with the particular knowledge, skills, and abilities to perform successfully the duties of this position, and that is typically in or related to the position. To be creditable, specialized experience must have been equivalent to at least the GS-04 grade level in Federal service. Examples of specialized experience include: group leader in community or residential recreation or training programs; counselor in a youth summer camp; instructor in a training program for the development of job skills; work that demonstrated the ability to be responsible for and to lead and train members of a job crew; group activities leader in the Armed Services; unit leader-monitor-proctor for a dormitory group in a boarding school or other residential facility; aid or assistant to professional or specialist staff in such fields as personnel (private or government), employment guidance in a vocational or academic institution, social service, or manpower development; aid or assistant to professional or associate staff engaged in work of a social services nature in private, public welfare, government program, or community organizations, or in voluntary activities; aid or assistant to professional or specialist staff working with individuals who are addicted to or abuse alcohol and/or drugs. NOTE: This experience must be detailed in your resume to receive credit.

Education Substitution (Transcript Required): Possess a bachelor's degree or 4 years of study leading to a bachelor's degree that included at least 24 semester hours in any combination of subjects directly related to the work of the position; that included a major in social behavior, human behavior, or health care, or; was supplemented by specialized experience in activities that afforded the opportunity to acquire and demonstrate the skills required by the position (gained, for example, in part-time or summer employment, in volunteer social welfare or recreation programs, or in community organizations).

Combination (Transcript Required): An equivalent combination of specialized experience and post high school education as described that demonstrates your ability to perform the duties of this position.

IMPORTANT: A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Physical Requirements: The work is primarily sedentary. Typically, the employee will sit to do the work. However, there may be some walking, standing, bending, carrying of items such as books, papers, groceries, household goods, etc., accessing transportation and driving government car or van.
