The Home Based Primary Care (HBPC) Program Director/Coordinator is a Registered Dietitian; he/she is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC, .

Duties

The Home Based Primary Care (HBPC) Program Director is a Registered Dietitian who executes position responsibilities that demonstrate leadership, experience, and creative approaches to management and coordination of complex client service provision. He/she facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Geriatrics and Extended Care Service and all healthcare disciplines included on the HBPC teams. The HBPC Program Director/Coordinator is responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations. Responsible for the development and continued effective functioning of the interdisciplinary health care team. Recognizes and utilizes the skills, knowledge and contributions of each team member. Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program Directors, Veteran Integrated Service Network (VISN) leadership and VA Central Office staff on a regular and ongoing basis. Assures the program is able to optimize cost effectiveness (budget) and patient outcomes. Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including: selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency. Assesses the continuing education needs of program staff and assures appropriate resources to meet those needs. Coordinates student education programs in HBPC. Responsible for the regular review and reporting of quality metrics. Instrumental in initiating and implementing quality improvement projects. Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA and VA owned Vehicles. Processes, assigns and tracks referrals for HBPC and ensures timely access to HBPC. Responsible for managing the Electronic Waiting List (EWL) for HBPC referrals. Collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Dietitian candidates must be proficient in spoken and written English in accordance with 38 U.S.C. § 7403(f). Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) - Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Current VHA Dietitians that qualify based on being covered by the Grandfathering Provision may not be promoted to a higher grade if they do not meet the current RDN/RD requirements. Grade Determinations: GS-13 Supervisory Dietitian (Facility Program Coordinator) Experience Requirement: In addition to meeting the basic requirements, to qualify for the GS-13 you must possess at least 1 year of experience as an RDN/RD equivalent to the next lower grade level (GS-12) which demonstrates the following Knowledge, Skills and Abilities: Knowledge of program administration to establish and modify program goals and objectives, coordinate activities, evaluate accomplishments and solve problems. Skill in developing quality improvement or research activities. Skill in developing training and reference materials. Ability to supervise subordinate staff (i.e., plan, organize, delegate, direct, control and review activities and performance). Ability to develop policies and procedures. Ability to provide program-specific coordination to include leadership, technical advice and staff development. Physical Requirements: The position often places the incumbent in a position of stress. Frequent/daily travel is required and overnight travel may be required when multi-day stays are necessary.
