THIS IS A NATIONAL GUARD TITLE 5 EXCEPTED SERVICE POSITION. This National Guard position is for a NURSING (CASE MANAGER), Position Description Number T5637P01 and is part of the CO State Surgeon Office, National Guard. The primary purpose of thisposition to facilitate medical readiness and medical retention for National Guard military members.

Duties

Organizational/Administrative Role: Interpret and translate SM limitations from non-military providers. Draft language in web-based system and submit recommendations for SMs with health limitations to profiling military provider for consideration and signature. Consultant/Collaborator Role: Provides advice, guidance, and health teaching to SM and Chain of Command, in individual or group settings designed to assist the SM in understanding, accepting and following medical recommendations. Educator Role: Coordinates and implements health prevention and education programs related to fitness for duty, medical readiness, and deployability. Professional Demeanor: Assumes responsibility and accountability for case management of all Soldiers in case load. Performs other duties as assigned.

Requirements

  • Must be able to obtain and maintain the required security clearance.

Qualifications

The ability to administer the case management of National Guard Service members to include identifying necessary changes, advising others of its requirements and acting as a liaison in providing information and education on the program. 2. Experience working with or knowledge in applying analytical and evaluative methods and techniques to review in order to identify case management issues, determine medical readiness of service members and make recommendations for improvement. 3. Knowledge and ability to use professional nursing clinical expertise to assess, evaluate, and recommend case management interventions. 4. The ability to communicate effectively in order to gather data, present information, coordinate and facilitate activities, and make recommendations both orally and in writing. 5. Basic knowledge of medical automation and its use in data collection and display.

Education

This position requires a bachelors with an RN or an RN with a certificate and 2 years of experience, or an LPN with minimum of 2 years of experience, and active state licensure or transferable license from other states.
