The Post-9/11 Military to VA (M2VA) Case Management Program Manager manages, directs, and oversees the M2VA Program for the VA Portland Health Care System catchment area. This includes managing all transition activities and the coordination of care and services for Post 9/11 service members and Veterans treated in the service area.

Duties include, but are not limited to: Administers information and analytical systems to evaluate and enhance the quality of service provided to Post 9/11 Veterans. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The incumbent manages a group of employees performing work at various GS levels and provides administrative, technical, and clinical supervision necessary for accomplishing the work of the program including outreach efforts and all CBOC involvements. Performs the administrative and personnel management function relative to staff supervised. Establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system. Observes workers' performance; demonstrates and conducts work performance critiques. Provides informal feedback and evaluates employee performance. Resolves informal complaints and grievances. Develops work improvement plans, recommending personnel actions as necessary. Provides advice and counsel to workers related to work and administrative matters. Effects disciplinary measures as appropriate to the authority delegated in this area. Reviews and approves or disapproves leave requests. Assures that subordinates are trained and fully comply with the provisions of the safety regulations. Supervises employees to accomplish the work of the organizational unit. Exercises supervisory authorities and responsibilities involving work assignment and review, as well as the administrative and personnel management functions relative to the staff supervised. The incumbent performs a wide range of services and provides technical assistance to state, local, and private agencies, and universities. Serves as the primary point of contact for Oregon National Guard Medical Command. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.). Participation on national social work workgroups or committees may be requested. Oversees and/or coordinates an outreach event for Post 9/11 active duty service members, Veterans, and their family members. Collaborates and provides outreach activities to VA, Veteran Service Organizations, military, and community agencies to provide information and education on issues related to readjustment for returning combat Veterans and VA services available to assist to readjustment. Performs work related to the review, monitoring, analysis, planning and assessment of various programs including the use of the post 9/11 Department of Defense Registry, Veterans Information System (VIS), Patient Treatment File searches, Care Management Tracking and Reporting Application (CMTRA), and electronic health record (EHR) records. Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit, such as National Guard Headquarters, community agencies, and CBOCs, under conditions that make day-to-day supervision difficult to administer. Ensures that severely-injured and/or or ill post 9/11 Veterans with identified case management needs are assigned to a nurse or social worker case manager and oversees or works closely with care management teams to include nurse and social worker case managers, the Spinal Cord Injury (SCI) Coordinator, the Polytrauma Case Manager, and the Women Veterans Program Manager. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice.
