The St. Louis VA Medical Center is seeking a highly qualified Maternity Care Program Coordinator to provide care coordination for our Veterans during pregnancy and post-delivery care at the VA St. Louis Health Care System. The Maternity Care Coordinator will be assigned to the Women's Clinic in Primary Care services and provide maternity care services to all Veterans across the facility.

The Maternity Care Coordinator (MCC) provides care coordination for our veterans during pregnancy and post-delivery care at the VA St. Louis Health Care System. The MCC is assigned to the Women's Clinic which aligns under Primary Care Services and provides maternity care services to all veterans across the facility. The MCC ensures effective communication between Veterans, VA facilities, and community providers who deliver the Veterans' maternity care services. The coordination of this care is critical to maternal safety to include coexisting mental health conditions, and continuity of care during the postpartum period. The MCC identifies mental and behavioral risks and other social determinants of health and connects veterans to resources as needed. The MCC will follow a multi-disciplinary approach to ensure effective collaboration between service lines including Primary Care, Gynecology, Medicine, Emergency Department, Nursing, Community Care, and contracted or affiliated hospitals/clinics proving care for our veterans. The MCC will provide specialized intervention and treatment modalities to include individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. The MCC's role is crucial to identifying physical, social, and/or, mental health barriers and assisting the veteran with prompt care coordination with our VA providers and non-VA obstetricians. The MCC tracks maternal and fetal outcomes to include complications, demographics, and social determinants. The incumbent works to facilitate veteran satisfaction, reduce duplication of services or delay of care, identify and provide appropriate resources/interventions and to monitor quality of care and services received. Additional duties include but are not limited to the following: The MCC ensures effective coordination of care between VA and maternity care providers in the community, including relevant VA and community specialist providers treating the pregnant veteran. The MCC contacts the veteran to initiate coordination of maternity care services that extend up to one year post-partum. The MCC follows the VA National Maternity Telephone Care Program. Routinely complete depression screenings throughout the veteran's pregnancy and post-partum period. The MCC-LCSW is qualified to complete the Comprehensive Suicide Risk Evaluation veteran following a positive Columbia Suicide Severity Rating Scale for management of mental health care services. Independently completes thorough maternity psychosocial assessments to identify social determinants of health and needs of the Veteran. Facilitates or co-facilitates with other VA specialty providers relevant mental health and support groups during the perinatal period. Present at designated meetings/committees to provide briefings on the status of the Maternity Care Program and educate staff on prenatal/postpartum needs. The incumbent will ensure that the VA's mission, vision, and delivery of health care remain a priority for our pregnant Veterans. Reviews the maternity care statistical data, identify areas for development, and facilitate clinical interventions in collaboration with facility Women's Health, VISN, and National leadership. Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with SW standards, Joint Commission standards, and facility standards. Provides clinical knowledge and consultation to colleagues regarding veterans reproductive health, pregnancy, and maternity care needs. Responsible for supporting the implementation of programs in relation to reproductive health and maternity care in relation to the Women's Health Program initiatives. Additional duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determination GS-12: Experience/Education. One year of experience equivalent to the GS-11 grade 12 level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program areas. (e) Ability to provide training, orientation, and guidance within clinical practice Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. Preferred Experience: Prior Maternity Care Coordinator and Women's Health Care experience.
