This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Provide case management for a caseload of Veterans in the ICMHR/MHICM program, many of whom have complex multiple causation in differential diagnosis and treatment and may be at elevated risk for suicidal thoughts, behavior, or active psychosis. Provide trainings to staff related to coding, CPRS documentation and best practices within the field. Supervise social workers for advanced practice licensure or the graduate trainee program. Act as a mentor for less experienced social workers, participate or chair committees or groups that impact social work practice and/or lead roll out of evidence-based practices. Act as surrogate to the ICMHR/MHICM Program Manager, for CPRS purposes, cosigning notes/consults/orders and providing clinical guidance Provide evidence-based and recovery-oriented practices as part of the ICMHR program to include Social Skills Training (SST), Illness Management and Recovery (IMR), Motivational Interviewing and Cognitive Behavior Therapy (CBT). Responsible for collecting and submitting ICMHR/MHICM participant data as outlined by NEPEC evaluation procedures. The incumbent is responsible for providing ICMHR/MHICM case management. Coordinate care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Participate in the ICMHR/MHICM Program conference calls and attend training offered by the National ICMHR/MHICM Program. Advise the supervisor of shifting trends with caseloads and identify the need for and make recommendations on program procedure or policy changes based upon work experiences. Participate in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Document all contacts with patients/families in the medical charts; completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies Work with VA Medical Center multi-disciplinary teams and community agencies to develop more effective and efficient ways of delivering healthcare services to ICMHR/MHICM Program Veterans. Provide a full range of social work services which includes case work, individual, family and group education/counseling, discharge planning, case management and other services. Responsible for the management, coordination of social work services to Veterans and their families in inpatient, outpatient, and community settings. Conduct psychosocial assessments to determine needs of Veterans and family members. Participate in the treatment planning process with other disciplines. Access Veterans on a regular basis, no less than quarterly and in accordance with local policy, for changes in their needs, acuity level, and preferences for care. Utilize psychosocial rehabilitation and recovery practices to promote Veteran engagement and self-efficacy. Participate in discharge planning with other disciplines as appropriate and ensure that discharge plans are executed in a timely and appropriate manner. Provide consultation/education to Veterans and families on community resources, advance directives, and VA benefits. Act as liaison between VA and community resources. Address Advance Directives and Organ Donation in accordance with the standards of their work area. Provide wellness/prevention education and facilitate patient/family support groups as appropriate. Maintain data and statistical compilations to comply with JC, CARF, VA and medical center policy and procedures. Assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness, and death. Document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. Participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency - You must be proficient in spoken and written English as required by 38 U.S.C. 7403(f) Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Driver's License - The position requires that the employee travels occasionally, requiring operation of a government vehicle. This position requires a valid state driver's license. Grade Requirements: Senior Social Worker, GS-0185-12 Experience/Education. The candidate must have at least two years of experience 8 post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure Requirement - You must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. In addition to the experience above, candidates must demonstrate all of the following Knowledge, Skills and Abilities. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. Preferred Experience: Experience working with Individual dealing with severe and persistent mental illness References: VA Handbook 5005/120, Part II, Appendix G39 Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation, if necessary, without endangering the health and safety of the applicant or others. Position requires walking throughout the community, the clinic, and the Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. This position requires frequent travel into the community, usually around 80% of time, to meet with Veterans. Must meet eligibility criteria to drive a GSA vehicle. Some travel is required to meet with community agencies and partners who provide assistance and support to the MHICM Program and its Veterans. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
