The Social Work Program Coordinator serves as as Program Coordinator of the Veteran-Directed Home and Community Based Services (VD-HCBS) Program which is one of VHA's innovative programs that promote patient-centered, non-institutional long- term care and is located at the West Haven CT VA Medical Center.

Duties

Duties of this position include but are not limited to: Responsible for planning, coordination, implementation, control, and evaluations of all aspects of the VD-HCBS program within Non-Institutional Care (NIC) Programs of the Social Work (SW) Service line of the Manchester Veterans Medical Center, and its associated Community Based Outpatient Clinics (CBOCs). Involved in planning, reviewing or revising policies and procedures that impact the VD-HCBS Program. May be required to do home visits as well as travel to CBOC's and community agencies as appropriate. Assures alignment with all VA policies and procedures and those of other (outside) regulatory agencies, such as the Office of Inspector General and The Joint Commission. Responsible for the establishment of provider agreements between the medical center and state agencies in order for the agencies to effectively administer the home and community based services to Veterans enrolled in the program. Organize and coordinate outreach orientations on the program for representatives from community agencies as well as professionals working at the medical center or in the CBOCs. Assure that budget and billing practices are maintained, adhering to national guidelines, as well as types of services to be bundled for program participants with state responsible agencies. Participate in research activities, National and VISN calls as they pertain to enhancement of participant-directed services covered under the program, while also being available to perform other duties as assigned. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Social Worker-Program Coordinator GS-12 Experience: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience with case management, long term care and skilled home care needs. Experience with VA program management, discharge planning and family dynamics. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-39 Social Worker Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others.
