This is a Direct Hire Open Continuous for Nurse (Community Health) for the Public Health Nursing at the Whiteriver Service Unit and will report to the Supervisory Nurse onsite. The Nurse (Community Health) plans, conducts and evaluates a comprehensive community health nursing program in an assigned area, and assists the community in planning and evaluating a local health deliver system.
Duties
Formulates and implements overall community health nursing programs and plans to satisfy varying patient load requirements, priorities for service and other healthcare objectives;
Tracks community health status and evaluate health trends and risk factors of population groups;
Collects data in a systematic organized manner and analyzes the collected assessment of data;
Investigates health problems and hazards such as communicable diseases and develop a recommended plan of action to address these issues;
Provides health education to individuals, families and community groups in various settings;
Performs community assessments to develop a plan of action; and
Collaborates and establishes partnerships with Tribal Community Health Representatives, Local, State, and Federal systems.

Qualifications
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
Note: Employees currently assigned to positions in the Nursing series, 0610 as of September 2017 will be considered to have met the basic requirements for the position occupied.
BASIC REQUIREMENT(S):Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in the specialty area of nursing.
In addition to the above Basic Requirements, applicants must meet one of the following requirements:
GS-7 Basic Requirements:
Possess a Bachelor's degree in Nursing and eligibility under the Superior Academic Achievement provision (https://www.opm.gov/policy-data-oversight/classification-qualifications/general-schedule-qualification-policies/#url=General-Policies).
OR
Possess a Bachelor's degree in Nursing and at least one (1) full year of professional nursing experience (equivalent to the GS-5 grade level): clinical nursing experience that includes planning, implementing, and evaluating a patient's health status to identify health promotion and disease prevention strategies; work with individuals, families and communities as a population focus and apply a systems level perspective to factors that impact health; provision of nursing care services and assessment in a variety of settings in homes, schools, clinics, community centers, jails, detention centers rural and remote Indian communities while focusing on improving population health in the environments where people live, work learn and play.
OR
Possess a diploma or associate degree in professional nursing and two full years of professional nursing experience: clinical nursing experience which includes planning, implementing, and evaluating a patient's health status to identify health promotion and disease prevention strategies; work with individuals, families and communities as a population focus and apply a systems level perspective to factors that impact health; provision of nursing care services and assessment in a variety of settings in homes, schools, clinics, community centers, jails, detention centers rural and remote Indian communities while focusing on improving population health in the environments where people live, work, learn and play.
OR
Completed at least one (1) full year of graduate education in a professional nursing educational program.
GS-9 Basic Requirements:
Possess a master's degree in professional nursing.
OR
Possess a diploma, associate degree or bachelor's degree in Nursing from a professional nursing program and have at least one full year of professional nursing experience (equivalent to the GS-7 grade level or above): working with individuals, families, and communities implementing and supporting community care coordination; and interacting with health care system providers on referrals and transitions in care; and collection and analyze data as part of community assessment, implementing case management processes and mentoring colleagues.
GS-11 Basic Requirements:
Successfully completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
OR
Possess a diploma, associate degree, bachelor's degree or master's degree from a professional nursing educational program and I have at least one full year of professional nursing experience (equivalent to the GS-9 grade level): Experience in applying nursing theory and public health science to preserving and promoting the health of the population with the nature of the practice being continuous and comprehensive; development and implementation of needed health services in the community to address the well-being of the population and serving as an advocate and facilitating change; Provision of public health program activities which are based on the community needs, program resources, and establishing goals and objectives; and, identification of vulnerable groups in a community by assessing the needs of the population and tailoring services to the special needs of high risk groups
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
