About the Position: This position is with the Defense Health Agency, located at Desmond T Doss Health Clinic, Pediatric Services at Schofield Barracks, Hawaii.
This is a Direct Hire Solicitation.
Duties
The incumbent of this position serves as a member of a multidisciplinary team to provide assessment, planning, implementation, coordination, evaluation, and monitoring of patients for health options and services.
Conducts a comprehensive assessment of beneficiary’s health needs in order to develop a plan of care.
Attends multidisciplinary meetings, identifies potential case management clients through health planning assessments, review of the multiple databases and points of contacts available for the specified population.
Screens all potential case management clients within established timeframe after identification for appropriateness/benefit of case management services to client and/or institution.
Interviews and counsels potential case management clients within established timeframe as to services needed to optimize current health/psychosocial status and benefits of case management services.
Provides guidance on establishing appointment protocols to match patients' needs with available resources in a timely manner.
Requirements
- One year probationary period may be required.
- This position has a Temporary Duty (TDY) or business travel requirement of 5% of the time.
- Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
- This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position
- Detection of the presence of drugs is warranted. This position is subject to a pre-employment screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.

- This position is subject to screening and update of required immunizations unless exempted for medical or administrative reasons in accordance with Army Regulation 40-562, Chapters 3 and 4.

In order to qualify, you must meet the education and experience requirements described below. Basic Requirement for Nurse (Clinical/Case MGMT):
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.)
Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-11 grade level or above) which includes educate patients and staff on patient care plans; identify issues with patient care plans and provide recommendations or alternate solutions; conduct evaluations of cases and establish the correct continuity care for the patient; provide advanced practice clinical nursing experience in managing the continuity of care for the beneficiary populations; conduct a comprehensive assessment of patient health needs in order to develop a plan of care; conduct extensive evaluation of case management patients. FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
