About the Position: This position is located in Schofield Barracks, Hawaii.
Incumbent serves as Nurse for Population Health Improvement, manages the health metrics and outcomes of the primary care population, coordinates the implementation of clinical practice guidelines through consultation and collaboration with healthcare providers, clinical and administrative support staff, and patients.

Duties
Serves as Nurse for Population Health Improvement
Manages automated population health web-based applications outcomes data to determine program needs.
Identifies resources to integrate Clinical Practice Guidelines into all clinical areas
Assists to standardize Disease Management across the patient care continuum by monitoring Disease Management of the patient in accordance with the clinical practice guidelines.
Works closely with the Nurse Case Managers and the primary care team to ensure patient-centered, whole-person concept within each encounter
Attends conferences, committee meetings related to Clinical Practice Guidelines and medical management activities.
Identifies strategies designed to improve patient access, improve adherence to population health metrics, and reduce administrative burdens for beneficiaries and improve the cost effectiveness of health care delivery.
Conducts population health and medical management program evaluation and feedback, interprets data and makes evidence based recommendations.
Requirements

- This position is a Testing Designated Position subject to applicant testing and random drug testing. Selectees will be required to sign DA Form 5019 requiring participation in random drug urinalysis testing.
- Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
- This position has a Temporary Duty (TDY) or business travel requirement of 5% of the time.
- Must obtain within 30 days of employment and maintain certification in Basic Life Support (BLS).
- Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series.
- This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination.
- This position has specific initial background investigation and periodic re-verification requirements since its duties require interaction with children and youth under the age of eighteen (18) years of age on a recurring basis.
- Must possess and maintain current licensure in a state, territory, or the District of Columbia.
- This position requires lifting up to 40lbs.

Basic Requirement for Nurse (Administration/Ambulatory):
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.).
Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below. Note: You will be required to provide appropriate documentation to the respective Human Resources Office to validate your status.
Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:
Education/Experience: A diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and possession of at least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) which includes collecting data to identify problems, assisting in developing or improving processes, and managing automated health care systems
OR
Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
Completion of all requirements for a doctoral degree (Ph.D. or equivalent) from a professional nursing program
OR
3 full years of progressively higher level graduate education from a professional nursing program
Education
FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
