The Social Worker COMPACT Act Coordinator is the primary navigator and subject matter expert for Veterans receiving care under the COMPACT Act. This includes coordinating transitions in care for Veterans receiving acute suicide care between varying venues, types (medical and mental health), and location of care (direct and community) in collaboration with assigned care coordinators at each step of the Veteran journey.

Duties

Duties include but are not limited to: Develop and monitor administrative and clinical plans for Veterans receiving COMPACT Act services. Oversee daily program operations; provide reports and metrics. Collaborate with local facility revenue and finance departments. Advocate for Veteran ensuring they do not receive COMPACT care related bills. Provide training, education, orientation and guidance on COMPACT Act and clinical program area as the facility Subject Matter Expert. Review and develop policies, standard operating procedures, practice guidelines for the program. Coordinate between health care providers and care managers in direct care and community care. Collaborate with interprofessional teams within the VA healthcare system and community to navigate complex care coordination. Consult and negotiate across program areas to ensure appropriate resources are secure in a clinically relevant and timely manner. Coordinate with local Patient Advocate Tracking System - Replacement (PATSR) COMPACT clinical review team. Provide clinical social work assessment, intervention, treatment and follow-up, using a social determinant of health framework. Assist with navigation between varying levels of care and conducts comprehensive transition planning. Utilize designated COMPACT Act templates and coding requirements when documenting in the medical record. Organize work, set priorities, meet multiple deadlines, and evaluate assigned program area. Supervise multidisciplinary staff (where appropriate). Understand psychosocial treatment and independently implement treatment modalities when working with patients, families and groups who are experiencing a variety of psychiatric, medical, and social problems utilizing individual, group, and family counseling skills to achieve treatment goals. Establish and maintain effective working relationships with clients, staff, and representatives of community agencies. Communicate both orally and in writing with people from varied backgrounds. Understanding of medical and mental health diagnoses, disabilities, and treatment procedures to include acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Complete the PHQ-I9, Comprehensive Suicide Risk Assessment, and/or Suicide Behavior and Overdose reports in Computerized Patient Record System (CPRS) on all patients with suicidal ideation and on any patient whose record is flagged "High Risk for Suicide." Use computer software applications for drafting documents, data management, and tracking. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grandfathering Provision: The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation. All persons employed in VHA in this occupation on the effective date of this qualification standard (September 10, 2019) are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Such employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. § 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Examples of specialized experience at the GS-11 include, but are not limited to: assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence; use of clinical social work skills; maintaining patient privacy and confidentiality per policies, handbooks or directives; acting as an advocate with appropriate community service providers and agencies when it serves the best interest of the patient and family members/caregiver; independently assessing the psychosocial functioning and needs of patients and their family members, identifying the patient's strengths, weaknesses, coping skills, and psychosocial acuity. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Prior experience in role, setting or working with population. Clinical experience, EBP certified. Clinical care coordination. Quality or utilization management experience. Prior Veterans Healthcare administration experience. Licensed to practice at independent practice level. The actual grade at which an applicant may be selected for this vacancy is above the full performance level at GS-12. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. Incumbent may carry and lift light items weighing less than 15 pounds.
