This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Mental Health Case Management (MHCM) program serves veterans with severe chronic mental illness. Services are recovery-oriented and are primarily delivered to veterans in the community. These services are delivered by an integrated, multidisciplinary team with a focus on a high intensity of care, crisis prevention and management, flexibility, and community-oriented setting. Further, MHCM focuses on rehabilitation through practical problem solving, crisis resolution, and adaptive skills building with transition to self-care and independent living where possible. Administrative: Directs the operations of the program through daily meetings with MHCM program staff to evaluate ongoing needs and assign administrative and clinical tasks as appropriate. Leads team staffing regarding patient selection and screening, determination of treatment plan and goals, progress evaluations, discharge planning and aftercare. Provides leadership with inpatient teams to assure continuity of care by providing consultation regarding clinical needs, treatment plans, and case management services for assigned patients. Works to promote appropriate treatment, rehabilitation, and discharge of patients consistent with program goals and with individual treatment plans. Prepares and submits reports in a timely fashion. Responsible for completing documentation of appropriate data in patients' medical records and is responsible for entering clinical and research data in the computer; also completes MHCM forms for evaluation and monitoring of service delivery and outcome. Supervises the training of social workers, social work associates, volunteers and students; participates as required in orientation, training and staff development programs. Research activities: As needed, incumbent collect clinical data for use in the evaluation of the MHCM program, using protocols developed by the program. Clinical: Provides comprehensive outpatient treatment with major emphasis on recovery- oriented principles in case management activities for psychiatric patients who are chronic users of inpatient resources. Provide clinical support and /or treatment to an adult or geriatric patient population on a regular basis. Utilizes individual, family, and group therapies and educational programs to provide psychosocial treatment and evaluation with documentation. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to SW Licensure Summary by State. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12: Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience working with Severe Mental Illness populations, specifically in case management role(s). References: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G39 Social Worker Qualification Standard. This can be found in the local Human Resources Office. The full performance level of this vacancy is GS-12. Physical Requirements: The work is primarily sedentary, however incumbent should be able to perform prolonged walking and standing necessary to conduct worksite visits, employer tours, etc. Incumbent must possess a valid driver's license in order to transport Veterans in government vehicle to interviews, tours, etc. with employers or other community resources.
