The Wound Care Nurse Practitioner/Clinical Nurse Specialist acts as a consultant to outpatient, inpatient, resident, primary care, home-based primary care, medical and nursing staff in assessing, planning, implementing, evaluating and reassessing the care of patients with a variety of wounds and stomas, to include but not limited to lower extremity wounds, abdominal stomas, draining wounds, fistula, pressure ulcers, dermal wounds, acute surgical wounds, and incontinence issues.

Duties

The Clinical Nurse Specialist(CNS)/Advanced Practice Registered Nurse (APRN) have prescriptive authority with a panel of patients. Core functions include diagnosing and treating acute and chronic health care problems, prescribing medication and treatments and is an independent practitioner providing quality and standard of care to a population of Veteran patients. Teaches health promotion and disease prevention to patients. Mentors and precepts students/trainees and those with lesser experience. Duties include but not limited to: Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Applies the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses, and attainment of appropriate lengths of stay and the effective level of care. Core functions include daily assessment of patient care needs to determine the most appropriate use of expertise in clinical coordination, patient and staff education. Demonstrates consistent clinical competence in providing nursing care. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care problems on the unit. Initiates and implements an interdisciplinary plan of care based on assessments. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. Initiates referrals to other disciplines and/or services as appropriate. Accurately documents in the medical record patient findings, assessments, and care provided. Documentation is technically informative, complete, accurate, consistent, concise, and timely. Effectively functions as the charge nurse, team leader, or in other leadership role that guides and directs others who provide patient care. Makes assignments and directs activities of other unit staff, (RNs, LPNs, students, PSAs, other team members). Uses a team approach to identify, analyze, and resolve patient care problems that result in improved patient outcomes.
