The Behavioral Health Nurse Case Manager is responsible for coordinating and supporting behavioral health, medical, and ancillary services. The Behavioral Health Nurse Case Manager plays a vital role in the providing health care coordination and wellness education to improve the patient care experience.

Duties

Demonstrates understanding of the nursing process as well as disease processes, including the ability to recognize physical, psychological, social, cultural, spiritual, and environmental needs. Interventions that promote and foster health, assess patient level of functioning, assist patients to regain or improve their coping abilities, maximize strengths, and prevent further disability. Conducts health assessments (including pain assessments for those with chronic pain), targets situations that put persons at risk, and initiates interventions Provide basic health care teaching to patients, family members or surrogates. Serve as primary point of contact for communication between the patient and the providers regarding patient data. Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Basic Requirements for the GS-11 (or equivalent) Grade Level In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples of Specialized Experience: Developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. Time In Grade (Merit Promotion Candidates Only) Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.

Education

This position has an education requirement. You are strongly encouraged to submit a copy of your transcripts or a list of your courses including titles, credit hours completed, and grades.
