The incumbent is a licensed pharmacist with prescribing privileges as outline in the individual's Scope of Practice, responsible for the daily professional and administrative operations of Clinical Pharmacy Services at the VA Southern Nevada Healthcare System. The incumbent may work evenings, weekends, and holidays and rotating shifts as priorities demonstrate a need delivering evidence-based comprehensive medication management.
Duties
Major duties and responsibilities include but are not limited to the following:
Clinical
Directly involved in design, implementation, and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas.
Acts as a provider with specialty training and scope of practice (as approved by Medical Center Professional Standards Board) providing expertise to other providers, pharmacists, nursing, and other disciples as appropriate.
Obtains medication histories from patients and summarizes significant finding in electronic health record (EHR) and/or to provider(s) as appropriate.
Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and appropriate therapeutic outcomes. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
Participates in multidisciplinary team rounds and provides drug information, and appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient outcomes taking into account consideration choice of therapy, safety, efficacy, formulary, and pharmacoeconomics.
Program Management
Acts as pharmacy representative to resolve staff/patient concerns.
Assists in drafting and updating of Pharmacy procedures and policies.
Assists with internal audits, medication use evaluations, adverse drug event reporting, drug regimen reviews, drug monographs, and other Pharmacy and Therapeutics (P&T) Committee, and qualify improvement-related activities as assigned.
Collects, analyzes, and acts upon customer service, workload, quality assurance, productivity, and workflow reports.
Coordinates pharmacy inspections, and formulates action plans.
Administrative
Assists Pharmacy Leadership in efficiently managing the facility's medication budget while maximizing service.
Actively serves and participates on local, VISN, and National committee as approved by Pharmacy Leadership and fulfills assignments timely and with quality products.
Assists with developing and establishing procedures to maintain compliance with TJC, Federal/State, OIG, DEA, VHA, VISN, and Medical Center requirements, regulations, standards, directives, handbooks, policies, and procedures.
Assists with planning short and long-range objectives, solves problems, and achieves results within budgetary and staffing constraints.
Attends and contributes to workshops, lectures, journal clubs, case presentations, in-service meetings, team meetings, particularly in area of specialty.
Performance Measures
Identifies new performance measures to improve quality of care.
Maintains current knowledge, responsibility, understanding, and oversight of Medical Center, VISN, and national performance measures in areas of assignment.
Provides and promotes cost-effective pharmaceutical services, offering reasonable and appropriate alternatives to high-cost/high risk drugs.
Reviews and uses dashboards to ensure compliance with performance measures and provides population-based management to meet measures.
Education/Training
Attends and actively participates in clinical pharmacy activities as assigned (e.g, staff development, journal clubs, case presentations, residency conferences).
Participates in initial/ongoing trainings and in-services for pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other Medical Center personnel as assigned.
Works with clinicians to identify, develop, and implement target programs. Develops and performs formal and informal education to promote these programs and gain acceptance.
Drug Information
Provides timely and accurate responses to drug information inquiries from all customers.
Serves as an authority on drug usage, interactions, dosages, and compliance to medical staff, nursing staff, technicians, and trainees.
Uses appropriate references to research drug information. Has a good working knowledge of information resources.
Research
Actively collaborates in design and implementation of research protocols, data collection, and analysis and interpretation of results.
Participates in collaborative research studies involving drug therapy with other services.
Supervises pharmacy resident and student projects as assigned.
Work Schedule: Monday to Friday, 8:00 am - 4:30 pm. May work evenings, weekends, holidays, and rotating shifts as priorities demonstrate.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.
Creditable Experience
(1) Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
(2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
(3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.
(4) Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.

English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Experience
In addition to the GS-12 requirements, applicants must have 1 year of experience equivalent to the next lower grade level.
Pharmacists assigned to this position must demonstrate the following Knowledge, Skills, and Abilities (KSAs):
(a) Ability to effectively supervise subordinate staff.
(b) Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
(c ) Expert understanding of regulatory and quality standards pertaining to pharmacy.
(d) Skill and managing people or programs.
Assignment
Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area.
Preferred Experience
The individual must be a licensed pharmacist with pharmacy residency training, supervisory and leadership experience. The individual must have current clinical experience and understanding of the goals, objectives, and performance measures associated with the development, implementation, maintenance, and improvement of clinical pharmacy programs to support quality patient care activities. Advanced degree (Pharm.D, M.S., MBA, MPH, or equivalent), PGY2 Pharmacy Residency, board certifications are preferred. The incumbent is responsible for daily professional and administrative operations of clinical pharmacy services at the VA Southern Nevada Healthcare System (VASNHS).

The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13.
Physical Requirements: This position requires visual acuity (depth perception and ability to distinguish basic colors and shades of colors), near vision correctable at 13 to 16, and far vision correctable in one eye to 20/20 and 20/40 in the other and keen hearing. This position requires potentially long periods standing (6 hours). The incumbent may be required to do light lifting and carrying of up to 15 pounds; reaching above shoulders. Both hands and legs are required along with the use of all fingers; hearing (aid permitted); repetitive computer use.
