This position is located in the Social Work Service of the Bay Pines VA Healthcare System. Incumbent will provide services at C.W. Bill Young Medical Center. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services.
Duties
Incumbent performs specialized treatment of complex physical or mental illness. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice within the PACT clinic and Homeless Programs.
Screening/Assessment In the area of screening and assessment, the Coordinator assists the Homeless PACT members in quickly triaging Veterans in the field, synthesizing available information along with the presenting problem, and distinguishing between brief and more in-depth responses. The social worker will develop a bio/psycho/social assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, available family members and significant others.
The incumbent must have an understanding of the problems of homelessness and respect for the diversity of the homeless Veteran population, being sensitive to human growth and development, age-appropriate needs and concerns of the homeless Veteran, and must be able to provide independent treatment of this Veteran population. The initial intake assessment will be completed at admission.
Incumbent will input and retrieve information for the assessment from the Veteran, collateral sources with appropriate ROI's and CPRS, and will complete an assessment through the Homeless Operations Management Evaluation System (HOMES) and other data collection tools as required. Due to the transient nature of our identified homeless Veteran population, the social worker completes re assessments on a quarterly basis and sees the Veteran as often as needed to meet the goals established in the initial intake plan.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Social Worker Program Coordinatorcandidates must have the following:
Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.

Preferred Experience: Working with the homeless population.
Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle.
