The Home Based Primary Care (HBPC) program provides cost effective primary care services in the home including palliative care, rehabilitation, disease management, and coordination of care.
The HBPC program targets frail, chronically ill Veterans who require care by interdisciplinary health care teams, continuity, coordination of care, and the integration of diverse services to cover complex medical, social, rehabilitative, and behavioral care needs. These Veterans need comprehensive, longitudinal home care services, as they age, to maximize function, minimize institutionalization, and maintain quality of life.
The HBPC Medical Director, appointed by the Chief of Staff, is the physician who is responsible for the overall medical care delivered by the HBPC team. The HBPC Medical Director position is administratively aligned under Geriatrics & Rehabilitation Service (G&RS). Overall supervision is provided by Chief, G&RS.
Position requires knowledge, skills, licensure and experience as a physician and knowledge and skill in the procedures, administration and documentation of inpatient and outpatient medical care, knowledge of emotional behaviors and responses to variety of treatment procedures. In addition, skill in maintaining effective work relationship with patients, members of the treatment team and other services in the organization and community partners is required
The HBPC Medical Director is responsible for:
1. Collaborating with the HBPC Program Director to:
a. Provide leadership to the HBPC Program at all locations-including CBOC'
b. Plan and direct the educational and clinical experience of medical students, residents and fellows assigned to Geriatrics & Rehabilitation Service/HBPC Section.
c. Assume a leadership role in the development and implementation of the HBPC performance improvement plan.
d. Advocate for HBPC with VHA leadership and the medical community.
e. Jointly select HBPC Team members.
2. Providing clinical input and oversight for all patient treatment plans.
3. Being readily available to the team members for collaboration when medical or other problems arise, responding within reasonable time.
4. Directly participating in HBPC team meetings via Microsoft Teams weekly. Attending one team meeting in person monthly at rotating locations.
5. Keeping the HBPC team apprised of medical care advances and practice standards.
6.Arranging physician coverage and communicating the plan of coverage to the HBPC team.
7. Collaborating with other HBPC Medical Directors, VISN leadership, and VA Central Office staff on program development issues.
Clinical Responsibilities
The HBPC Medical Director, in collaboration with the HBPC interdisciplinary team, is responsible for:
1. Providing clinical input and oversight as needed to HBPC providers.
2. Assuming medical oversight for HBPC patients and completing one home visit per month with HBPC provider.
3. Participating in treatment team meetings and covering treatment team meetings for planned and unplanned absences and vacancies of HBPC providers.
4. Serves as member of Medical Staff and any other assigned hospital committee, communication and sharing information with HBPC providers.
5. Providing clinical oversight for HBPC mid-levels at Community Based Outpatient Clinics (CBOCs).
6.Providing clinical/administrative support such as approving forms requiring physician signature for the HBPC patients, such as home care, DME, home oxygen, etc.
7. Direct responsibility and participation of Joint Commission (JC), Office of Inspector General (OIG) or any other outside surveyors.
In addition, the HBPC Medical Director will be responsible for:
1. Acting as the medical provider for Geriatric Clinic once a week. This clinic provides comprehensive interdisciplinary evaluation and management of Veterans with complex medical problems and/or dementia.
2. Providing medical coverage as needed in dire/emergent coverage within Community Living Center.
3.Collaborating with community home health and hospice agencies as needed for HBPC patients and signing off on prescribed therapies as medically appropriate.
4.Developing and updating policies, procedures and guidelines pertinent to HBPC service.
5.Taking full responsibility, development and leadership of policies, procedures and action plans that may arise during a pandemic such as COVID- 19 or other unforeseen or foreseen disaster or mass event.
6.Providing supervision for mid-level practitioners that are assigned to HBPC and completion of annual evaluation/performance appraisal of HBPC providers.
7.Following HBPC Staff Safety procedures.
8.Complying with Medical Center and VHA regulations regarding the use of government vehicles.
9.Demonstrating work practices that include adherence to Infection Control standards and the safe use and operation of equipment.

Qualifications
To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR
[(2) Those approved by the American Osteopathic Association (AOA),OR
(3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs.
Proficiency in spoken and written English.
Additional Requirement: Maintaining a valid Drivers' License, unrestricted medical license, and DEA registration.

Education
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
- Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
- Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
- For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
