This Supervisory Medical Care Transpiration Technician - Supervisory Paramedic position is assigned to the Department of Veteran Affairs, Veterans Health Administration (VHA), Veterans Transportation Services (VTS), Ambulance Service, in the Cincinnati VA Medical Center (VAMC) locations with an emergency room, acute care or urgent care clinic, or other similar departments performing basic and advanced life support procedures.

Duties

The Supervisory Medical Care Transportation Technician performs technical duties as a Paramedic (Basic and Advanced Life Support Services). In addition to performing all the duties and responsibilities of a Paramedic, the person in this position will, as a regular and recurring part of their assignment, exercise supervisory authorities and responsibilities over the Ambulance Services Program (ASP). The major duties include but are not limited to: Oversees all aspects of the Ambulance Services Program (ASP) Performs the full range of supervisory responsibilities to include but not limited to creates, adjusts and approves ASP employee schedules as well as reviewing and approving leave requests; interviews candidates for positions and recommends appointment, promotion or reassignment; observes performance and develops performance standards resolves formal and informal complaints; prepares and recommends awards or bonuses; provides advice and counsel related to work and administrative matters; assures subordinates are trained, etc.; Prepares forecasts of funds needed for equipment, materials and supplies Responds to on-site accidents and other emergencies involving injury and illnesses Determines and prioritizes appropriate medical treatment/procedures Performs patient triage and records patient's vital signs Administers basic and advanced life support care at emergency scene in accordance with established medical standards Provides fluid resuscitation by IV and manages trauma injuries Operates or utilizes various emergency vehicles and emergency/medical equipment Follows safety procedures, utilizes safety equipment and monitors work environment to ensure safety of employees and other individuals Operates the emergency transport vehicle and uses telephone and radio communications equipment and systems Assists in lifting and carrying patients into or out of transport vehicles and into treatment facility Performs all other duties as assigned. Work Schedule: Monday-Friday 8:00 AM- 4:30PM Condition of continued employment: Paramedic shall obtain certification as a National Registered Emergency Medical Technician (Paramedic) no later than (NLT) 18 months after employment as condition of continued employment with VA. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 06/04/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-11 position you must have served 52 weeks at the GS-09. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Individual Occupational Requirements Specialized Experience (for positions at GS-4 and above): Positions in this series range widely in type and include support duties to medical or health personnel such as audiologists, speech pathologists, medical officers, and optometrists. Therefore, technician experience is experience that required application of the knowledge, methods, and techniques of the position to be filled. The Supervisory Paramedic will oversee/monitor all aspects of the Ambulance Services Program (ASP). Evaluates procedures/techniques of patient care for compliance with emergency medical service protocols. Plans the work of the ASP employees, setting and adjusting both short and long-term priorities and goals. Creates, adjusts, and approves ASP employee schedules as well as reviewing and approving leave requests in accordance with VA policy and ASP workload needs. Incumbent will prepare forecasts of funds needed for equipment, materials, and supplies of the ASP. Incumbent shall provide advice on emergency medical service care, system delivery, and on the interpretation and application of policies, regulations, and directives. Develops, implements, and/or recommends organizational policy, planning, and procedural initiatives related to Ambulance Services. Takes corrective action to improve the quality and quantity of work and resolves problems not covered by precedents or established policies related to Ambulance Services. Incumbent ensures the operation of an effective system of internal controls by periodically reviewing internal control measures, initiating needed revisions, and evaluates procedures/techniques of patient care for compliance with emergency medical services protocols. In addition to the supervisory duties highlighted above, the Supervisory Paramedic performs all the duties and responsibilities of a Paramedic. Incumbent must the have the knowledge and ability to supervise, lead, and perform the following duties/activities: Responds to on-site accidents and other emergencies involving injury and illnesses, sometimes encountering situations which may be life threatening. Determines and prioritizes appropriate medical treatment/procedures and need for additional assistance. Supervisory Paramedics shall perform patient triage and take/records patient's vital signs as needed. Incumbent performs physical examinations and obtains patient history, assesses status of patient's neurological system, cardiovascular system, circulatory system, skeletal system, orthopedic system, motor and sensory skills, and level of consciousness. Supervisory Paramedics shall have broad knowledge and skills sufficient to provide and gather information on the human body structure, function, illnesses and diseases, and the treatment procedure supporting patient charting and goals for the assigned patient for the purpose of communicating with the patients and nursing/medical staff members. Paramedic Minimum Psychomotor Skill Set: Airway and Breathing Minimum Psychomotor Skill Set: BiPAP/CPAP, needle chest decompression, chest tube monitoring, percutaneous cricothyrotomy2, ETCO2/Capnography, NG/OG tube, nasal and oral, endotracheal intubation, airway obstruction removal by direct laryngoscopy, PEEP. Assessment Minimum Psychomotor Skill Set: EKG interpretation Interpretive 12 Lead. Pharmacological Intervention Minimum Psychomotor Skill Set: Central line monitoring, IO insertion, venous blood sampling, techniques of Medication Administration (Endotracheal, IV push and infusion), NG, rectal, IO, topical, accessing implanted central IV port, administered meds, Physician-approved medications, maintenance of blood administration, Thrombolytics initiation. Emergency Trauma Care Minimum Psychomotor Skill Set: Morgan lens. Medical/Cardiac Care Minimum Psychomotor Skill Set: Cardioversion, manual defibrillation, TC external pacing. Ambulance/Transport Vehicle Operation (25%): The Supervisory Paramedic selects the most efficient route to and from sites, taking into consideration traffic patterns, health facility locations, etc. Operates the emergency transport vehicle and uses telephone and radio communications equipment and systems. Incumbent shall have skill and familiarity with emergency and patient transport vehicle characteristics to safely operate the vehicle under adverse weather and/or traffic conditions to provide emergency and patient transportation. Shall have skills to read highway maps, program GPS devices, read and follow instructions in order to locate and complete assigned tasks.
