This position is for a Physical Therapist (PT) in the Veterans Health Administration (VHA), located in Eastern Colorado Healthcare System within the Physical Medicine and Rehabilitation Services. The primary function of this position will support Primary Care through PT embedded in PACT with same day access, walk-in clinics, and scheduled PT appointments.

Duties

Major duties include, but are not limited to: Clinical-Pertaining to the Orthopedic Clinical Position at the local VA PT and Patient Aligned Care Team (PACT) clinic and patients with primary musculoskeletal orthopedic deficits: Receives patient caseload through referrals from medical providers within ECHCS PACT clinics. Evaluation includes but is not limited to chart review, medical history, functional tests and measures, pain scale, social needs, and rehabilitation potential, patient and therapy goals with time frames, using standard and non-standard measures to assess the level of patient function. Develops and implements a treatment plan to meet the individual treatment needs. Treatment plan would be as described above. Keeps the medical team providers informed regarding outcomes of treatment through a variety of communication means. Documents in Computerized Patient Record CPRS for each patient by established policy including but not limited to: consult screening results, potential goals, expected outcomes in terms of function, and/or discharge summary including outcome of goals, as well as equipment provided, and educations given to patient and family. Complies with requirements to maintain prosthetic equipment supplies, providing necessary documentation of equipment provided to patients and obtaining stock replacement of equipment issued. Carries a full patient caseload, accepting new patient consults as referred by medical providers. Caseload will be inpatient and or outpatient Physical Therapy referrals and or Direct access/ direct scheduled patients. Other duties as assigned. Considers age and developmental level, gender, education, cultural background and socioeconomic, medical and functional status when providing Physical Therapy services. Is aware of signs and symptoms of abuse or neglect and reports to the appropriate authorities. Therapy services will be provided to an adult population of patients. Perform in the physical therapy program with complete independence and is responsible for maintaining high professional standards. The therapist displays courtesy, civility and constructive behavior, and expresses empathy for the legitimate needs and concerns of others. The therapist will meet the needs of customers while supporting the VA missions. Responsible in helping triage of all consults received and working with Medical Support Assistant (MSA) to schedule into multiple clinics as needed. Works to increase access to care through the use of walk-in clinic partnering with local PACT teams and providers. Education and Experience: Meets all of the regulatory and statutory requirements to practice physical therapy including graduation for an accredited university and the possession and maintenance of a current license to practice physical in one state of the United States as stated above. Completes annual mandatory training classes by hospital policy and practices the information learned to correct deficiencies and maintain a safe environment. Assists the SCCE in arranging and providing educational experiences for Physical Therapy students that affiliate with this facility, providing a suitable learning environment for the student by assisting the student when necessary and maintaining communication with the Physical Therapy Advisor from the Physical Therapy program. The therapist assists with the assessment of the student's performance to evaluate capabilities to function as a Physical Therapist. Attends Continuing Education for Physical Therapists annually to maintain a high level of professional competence, knowledge and skill, learn new treatment techniques, and explore new theories. Shares the knowledge with Physical Therapy staff. Administrative: Responsible to the ECHCS local Supervisor of Physical Therapy/Occupational Therapy in the department of Physical Medicine and Rehabilitation Services (PM&RS). Although the therapist is expected to make individual patient decisions with autonomy, the therapist is expected to perform as part of the overall PM&RS team. Participates in Program Improvement work groups and has a working knowledge of Departmental Program Improvement projects. Participates in multidisciplinary meetings as appropriate. Participates in PM&RS staff meetings and Physical therapy staff meetings as established. Participates in The Joint Commission and CARF Accreditation activities as required. Accepts flexible tour of duty that may include hours outside of the traditional 8:00-4:30 timeframe, may also include weekend hours and holiday coverage. Provides program development in PACT initiative at a local VA Clinic or medical center. Qualifications

Basic Requirements: Citizenship. Be a citizen of the United States. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed. Education or Experience. The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. Licensure. Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GS-13 Grade Determinations: Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following: Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; Or Board Certification in the specialty area recognized by the ABPTS; Or Completion of a fellowship program; Or An additional advanced degree in a related field. In addition, the candidate must demonstrate the all following KSAs. Demonstrated Knowledge, Skills, and Abilities. Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy. Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i). Ability to teach and mentor medical center staff in a specialized content area of physical therapy. Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2. i.). Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety. Clinical Specialists spend at least 25% or greater of their time working with specialized population(s) within their department. Physical therapists at this level perform assignments in specialty areas such as geriatrics, neurology, cardiopulmonary, orthopedics, spinal cord injury, amputee, wound care, chronic pain, electromyography, women's health or pelvic health, polytrauma, brain injury, lymphedema, vestibular, assistive technology, seating and mobility. The individual is assigned responsibility for serving as the subject matter expert in the content specialty area and as a consultant to physical therapy and other medical center staff in evaluating and treating patients in the specialty area. Clinical Specialists have advanced knowledge and demonstrate expert clinical practice skills in providing assessment and treatment interventions across the continuum of care. He/she utilizes advanced knowledge and expertise to modify treatment plans and to identify therapeutic activity interventions based on the changing needs, goals and performance of patients. Clinical Specialists have advanced training in specialized technology related to their area of practice. He/she establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis. He/she exercises expert professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge from inpatient or outpatient rehabilitation programs. If the medical center has a PT residency program, the clinical specialist may be on faculty for the program. Preferred Experience: Experience in a Primary care setting and/or urgent care setting required. Preferred to have at least 3 years experience as a Physical Therapist. References: VA Handbook 5005, Part II, APPENDIX G-12 Physical Requirements: Pre-placement and periodic physical examinations are required for hybrid occupations to ensure workers are placed in positions where they can perform the essential functions of their job considering their physical, mental and emotional capacities, without endangering their health or the health of their co-workers.
