The primary purpose of this position is to provide case management and care coordination that involves patient assessments, coordination of care, integration of services, and follow ups and evaluation processes for patients. These positions are located at the Chinle Service Unit - Chinle Comprehensive Health Care Center, Division of Nursing Services at Chinle, Tsaile and Pinon, Arizona.

Duties
Coordinates specialized care services to referred High Risk and At Risk.
Develop, maintain, and monitor chronic disease for patients requiring specialty services.
Serve as point of contact to internal and external multidisciplinary team.
Acts as liaison with Purchased Referred Care for referred appointment scheduling and follow up.
Evaluate outcome of action and care plans by making changes as needed.
Consult, teach, counsel, and support preventive, curative and rehabilitative measures.
Provides nursing care to patients in all age groups and involves patient's family in care.
Educate community-related issues, needs, and details care in the form of presentations and informational sessions.
Qualifications

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Basic Requirements for the GS-10 (or equivalent) Grade Level:
In addition to the above requirements, applicants must meet one of the following requirements:
Possession of a master's degree in professional nursing; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program. Examples of specialized experience: communicates and collaborates patient's information with patient and health care providers by providing adequate patient education. Care management involving patient assessment, initiating, and maintaining individualized multidisciplinary plan of care involving patients and their families.
Basic Requirements for the GS-11 (or equivalent) Grade Level:
In addition to the above requirements, applicants must meet one of the following requirements:
Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-10 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples of specialized experience: communicates and collaborates patient's information with patient and health care providers by providing adequate patient education of disease process/prevention. Evaluates for patient referral by determining degree of care. Care management involving patient assessment, initiating, and maintaining individualized multidisciplinary plan of care involving patients and their families by operating specialized medical equipment to give optimal care.
