Serves as Management & Program Analyst, providing support to Primary Care Service Line operations at a Health Care System (HCS) within VISN 23 (V23). The incumbent of this position is located under the Administrative Officer/Health System Specialist (Primary Care) and has primary responsibility as the Patient Centered Management Module (PCMM) Facility Coordinator for all Primary Care services with the V23 HCS and associated Community Based Outpatient Clinics (CBOCs).

Duties

***THIS IS NOT A VIRTUAL POSITION, YOU MUST LIVE WITHIN OR BE WILLING TO RELOCATE WITHIN A COMMUTABLE DISTANCE OF THE DUTY LOCATION*** This position involves a multi-grade career ladder. The major duties listed below represent the full performance level of GS-9. At the GS-7 grade level, you will perform assignments of a more limited scope and with less independence. You will progressively acquire the background necessary to perform at the full performance level of GS-9. Promotion is at the discretion of the supervisor and is contingent upon satisfactory performance, availability of higher level work, and availability of funds. Principal Duties and Responsibilities: Serves as PCMM Facility Coordinator for Primary Care Teams within a V23 HCS and its associated CBOCs. This includes the VA staffed and contracted CBOCs. The incumbent has primary responsibility for management of PCMM software program; performs management, and workload and program analyses. Independently responsible for ensuring the integrity and accuracy of the PCMM database, which is used to organize and manage the HCS primary care network and is reported directly to the VISN monthly. Works with clinic support staff, clinic managers, clinical service chiefs, CBOC staff, VISN staff and headquarters staff to maintain the integrity of this program and data. Provides input to VISN and national program managers on process challenges, program issues, and other problems so that these items can be addressed and corrected/improved. Uses this information to make recommendations to Primary Care and Home-Based Primary Care leadership regarding clinic access issues. The incumbent assures that Change of Provider forms are completed timely. Assists operating officials in implementing identified improvement actions and plans through participation in organizational improvement work through station programs such as Systems Redesign projects. Ensures facility PCMM standard operation procedures (SOP) or Directives are in place and current; meeting all local requirements and stakeholder concurrence to meet VHA PCMM goals and objectives. Uses performance data to identify areas for process improvement projects and works with Primary Care and clinic site leadership to identify any necessary resources for the successful implementation and management of such programs. Performs organizational and/or manpower unitization studies of the clerical and technical aspects of the Primary Care Patient Aligned Care Team (PACT) clinics with related functions to identify and develop improvements and efficiency of manpower and methods to present to leadership. Performs other related duties as assigned. For GS-7: Superior Academic Achievement at the GS-7 level:(SAA) at the baccalaureate level in the coursework detailed above is fully qualifying. Applicants who wish to apply under this provision must submit proof from their educational institution that they meet one of the criteria listed below. To qualify for SAA, applicants must have completed all the requirements for a bachelor's degree in a curriculum that is qualifying for the position to be filled and meet one of the following: Rank in the upper one-third of your class in the college, university or major subdivision. A grade-point average of "B" (a GPA of 2.95 or higher out of a possible 4.0) for all completed undergraduate courses, or those completed in the last two years of undergraduate study. A grade point average of "B+" (a GPA of 3.45 or higher out of a possible 4.0) for all courses in your major field of study, or those courses in your major completed in the last two years of undergraduate study. Membership in a national honor society (other than freshman honor societies) recognized by the Association of College Honor Societies. (Transcripts Required) OR Possess at least one full year of graduate level study or possess a master's or higher degrees in a field that provided the knowledge, skills, and abilities necessary to do the work. (Transcripts Required) OR Possess at least one year of work experience in healthcare delivery policies and procedures to many different clinical functions; understanding of clinical practices as they relate to patient care clinic creation and maintenance; working with Microsoft Excel, Access, Word, and Power Point software applications; applying management principles and techniques of analysis and evaluation, along with standardized clinic operational practices and procedures to conduct studies of clinic work processes. OR Possess a combination of successfully completed graduate education and experience to meet total qualification requirements for the grade level. To be computed by first determining the total qualifying experience as a percentage of the experience required for the grade level; then determining the education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level. (TRANSCRIPTS REQUIRED) OR Minimum Qualification Requirements: For GS-9: Possess at least one (1) year of specialized experience equivalent to the next lower grade level of (GS-7) in Federal service. Specialized experience is possessing of healthcare delivery policies and procedures to many different clinical functions; understanding of clinical practices as they relate to patient care clinic creation and maintenance; working with Microsoft Excel, Access, Word, and Power Point software applications; applying management principles and techniques of analysis and evaluation, along with standardized clinic operational practices and procedures to conduct studies of clinic work processes in various parts of an organizations to identify, analyze, and recommend solutions to problems in organizational structure, staffing, administrative procedures, work processes, or workload distribution. OR Possess a master's or equivalent graduate degree or 2 full years of progressively higher-level graduate education leading to such a degree or LL.B. or J.D., if related. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent 1 year of full-time study. If that information cannot be obtained from the school, 18 semester hours should be considered as satisfying the 1 year of full-time study requirement. (TRANSCRIPTS REQUIRED) OR Possess a Combinations of successfully completed post-high school education and experience may be used to meet total qualification requirements for the grade level. To be computed by first determining the applicant's total qualifying experience as a percentage of the experience required for the grade level; then determining the applicant's education as a percentage of the education required for the grade level; and then adding the two percentages. The total percentages must equal at least 100 percent to qualify an applicant for that grade level. Only graduate education in excess of the amount required for the next lower grade level may be used to qualify applicants for positions at grades GS-9. (TRANSCRIPTS REQUIRED) English Language Proficiency: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position.
