This position is located in the Outreach Branch of the Department of Preventative Medicine, in Whiteriver, AZ. The Outreach Clinical Nurse plans, conducts and evaluates a comprehensive list of community-based outreach services that serve as direct and indirect extensions of existing services elsewhere in the Service Unit. Additionally, the Outreach Clinical Nurse works in parallel, in collaboration, and at times as back up to Community Health Nurses in the same department.
Duties
Conducts epidemiological and community surveys, field investigations and research studies to appraise health needs.
Assesses, diagnoses needs and sets up a plan of action to meet these needs by providing health services to communities, families and individuals.
Plans the Outreach program in collaboration with the Tribal, State and County Health agencies, Tribal Health Board and other community organizations in planning health related activities. i.e.. The Inter-Tribal Council of Arizona's health outreach programs on Maternal Child Health.
Incumbent will actively participate in providing education to the service population regarding the IHS and the SU's mission and service availability to the various communities.
Makes physical and psychosocial assessments of neonatal, pediatric adolescent, adult and geriatric clients and is able to identify the range of conditions from normal to common abnormalities
Qualifications

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
GS-11 Grade Level: Your resume must demonstrate at least one (1) year of specialized experience equivalent to the GS-09 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Served as a Clinical Nurse performing professional nursing duties for the care of patients (ambulatory care outpatients and outreach clinical services) providing clinical care in patient homes or other community-based locations; Independently plans and evaluates patient status and patient's standard of care and revises necessary to meet the revisions in the medical care plan and the patient's changing needs; interview patient and/or family members and consults with attending physicians to determine the type of health and physical care needed; Participate in the development of nursing procedures and performance improvement standards for outpatient care medical services; Initiate patient referrals when necessary to maintain continuity of patient care.
