The medication history pharmacy technician is a clinical pharmacy technician with the ability to support the medication reconciliation and medication to bedside delivery program at the Charles George VAMC.

The duties for the Pharmacy Technician (Medication History) includes, but are not limited to the following:
Interviews patients, family members and/or caregivers, to develop an accurate list of medications, drug strengths, dosages, routes, and instructions the patient has been taking.
Contacts other pharmacies, hospitals, and long-term care facilities to complete the accurate list.
Assesses patient compliance with prescribed therapy through patient/caregiver interview and review of refill dates.
Compares the compiled list with the active medication profile in the computerized medical record of the patient.
Documents the compiled list in the electronic medical record, noting any discrepancies or omissions within 24 hours of the patient encounter.
Notifies the overseeing pharmacist of any discrepancies, omissions, questions or concerns regarding the patient.
Prioritizes operations and assigned clinical functions to provide the highest quality of patient care.
Reports all potential patient or pharmacy distribution problems verbally and/or writing as requested by the pharmacist in the area assigned.
Reviews medication profiles for prescription duplication, therapeutic duplication, prescription noncompliance, and allergies / adverse drug reactions. Reports identified patients to assigned pharmacist for further evaluation.
Reviews laboratory monitoring reports for selected disease states, identifies patients with lab values beyond normal limits, and reports these patients to the assigned pharmacist for further evaluation.
Coordinates the delivery of discharge outpatient prescriptions and associated updated patient outpatient medication list to the patient beside prior to patient discharge.
Provides education for patients concerning operational issues, need for therapy compliance or refill processing.
Documents clinical and operational activities as assigned.
Participates in quality improvement processes and programs as assigned.
Work Schedule: 7:00a-3:30pm, 8:00am-4:30pm, 9:00am-6:00pm, rotating weekends

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Pharmacy Technician candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Certification: Must pass a national certification exam and hold an active national certification through either: Pharmacy Technician Certification Board (PTCB), Certified Pharmacy Technician (CPhT) OR National Health-Career Association (NHA), Certified Pharmacy Technician (ExCPT).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-8
Experience: Candidates must possess one year of experience equivalent to the next lower grade level, in addition to meeting the demonstration of the following KSAs:
(a) Comprehensive knowledge of concepts, principles, methodology and policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.) Note: See Specialized Area.in the Definitions Section.
(b) Skill in training and orienting new and existing pharmacy facility employees and students on proper policies and procedures.
(c) Ability to evaluate, analyze and coordinate workflow and work activities within a specialized area or section of the pharmacy.
Certification: Must pass a national certification exam and hold an active national certification through either:
(a) PTCB as a CPhT. OR
(b) NHA as a ExCPT.
Preferred Experience: Obtaining and documenting patient medication history, assisting with medication reconciliation, and delivering discharge medications to the patient beside, laboratory value monitoring, medical chart/action profile poly-pharmacy reviews, medication utilization evaluation monitoring, pharmacoeconomic reviews, telephone logistical support, and supplemental patient education communication.

The full performance level of this vacancy is GS-8. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-8 to GS-8.
Physical Requirements: The position requires frequent walking and standing; repeated bending and prolonged sitting; moderate lifting and carrying (15-44 pounds); the ability to read handwritten or typed characters with or without glasses; ability to hear conversation voice with or without aid; and emotional and mental stability.
