ORGANIZATIONAL LOCATION: Department of Health & Health Human Services (HHS), Indian Health Service (IHS), Southwest Region, Phoenix Area -Resource Management Services- Purchase Referred Care Department , CRSU, Parker Indian Health Center. Position is located in Parker, Arizona and Supervised thru Supervisory Health System Specialist . Primary function is to be a clinical liaison for the purchase referrals and work with a team of providers to ensure that services are provided to patients.
Duties
Major Duties:
Clinical liaison and resource for Purchase referred care for clarifying provider referrals/orders; assisting with one care coordination of ordering complex durable medical equipment and care managing unsourced patient/ referrals.
Facilities communications with clinical staff ,and partners with the clinical care teams to ensure referrals are appropriately and timely; Team leader for Outpatient Case management and Purchase referral data base, ensures tracking process of referrals are appropriate and meet patient centered medical home guidelines.
Co- manages patient care needs with outside agency case managers and/or social workers of hospitalized patients who use Purchase referred care as their primary care network.
Manages and triages case management consult requests for al ambulatory clinics. Attends Purchase referred care team meetings and medical home strategic planning groups to provide updates regarding process improvements.

Qualifications

BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS - GS-11:
One (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Experience in providing care coordination; collaborating with multidisciplinary health team members for patient care planning; serving as a liaison between patients and healthcare providers and between healthcare facilities; evaluating and providing patient and family health education needs; facilitating transitions in patient care, and working with healthcare referrals that involve eligibility requirements such as the IHS Purchased and Referred Care program or Medicaid. OR
PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
